Interventional Radiology, 2nd ed (in 2 vols), edited by Wilfrido R. Castaneda-Zuniga and S. Murthy Tadavarthy, 1,484 pp, with illus, $240, Baltimore, Williams & Wilkins, 1992 Numerous advances in interventional radiology have occurred since I reviewed the first edition of this book in 1988. This expanded two-volume text has captured the state-ofthe-art information in the continuously developing field of interventional radiology. Although the entire first volume and a portion of the second volume are dedicated to the vast array of vascular interventional techniques, the second volume also includes interventional radiology in the pediatric age-group as well as a thorough review of modern uroradiologic and biliary radiologic interventions. Technical discussions of the gastrointestinal, skeletal, chest, and breast systems have been added. Overall, the sections on vascular interventional radiology are practical and useful. The text begins with a thorough review of embolotherapy and then carefully blends its specific techniques with unusual applications. Gastrointestinal bleeding and percutaneous transluminal angioplasty are completely addressed. Modem, up-to-date, yet still controversial techniques of percutaneous atherectomy follow. The next chapter, on the latest developments in intravascular stents, is provocative. Chapters on fibrinolytic therapy, mechanical thrombectomy, and use of inferior vena cava filters follow and complete the first volume in an orderly fashion. The second volume has stimulating chapters on removal of intravascular foreign bodies, vascular procedures in the pediatric age-group, and interventional treatment of impotence. The lengthy chapter on interventional uroradiology describes every possible technique ever conceived or performed in the urinary tract system. As an excellent complement to the previously mentioned chapters, a brief chapter on fallopian tube catheterization for diagnosis and treatment immediately follows. Conventional interventional techniques in the biliary tract are well described; however, some

repetition of the interventional instruments also used in the urinary system is evident. Nonetheless, this repetition is useful and actually emphasizes the use of basic tools (needles, guidewires, and catheters) by interventionalists in a wide spectrum of situations. The editors have achieved their goal of including all possible areas of percutaneous intervention with the addition of the chapters on breast biopsy and percutaneous venous access techniques. This encyclopedia of interventional radiology is easy to read and understand. The radiographic reproductions, diagrams, and photographs of equipment are extremely instructive. Each chapter is well referenced by the individual authors. This text is a must for the professional library of interventional radiologists. It should be a standard reference source for all residents in radiology as well as for surgeons and internists. Child and Adolescent Psychiatry: A Comprehensive Textbook, edited by Melvin Lewis, 1,282 pp, with illus, $120, Baltimore, Williams & Wilkins, 1991 Any textbook that includes the word "comprehensive" in its title risks "falling short" of the definition of that word. Dr. Lewis succeeds with this book, however. In the preface, he indicates that this text covers the field of child and adolescent psychiatry completely, an accomplishment that by any definition is a herculean task. The book is organized into nine sections: Normal Development, Development of Symptoms, Overview of Etiologic Influences, Nosology and Classification, Diagnostic Assessment, Syndromes, Treatment, Child Psychiatry and Allied Professions, and Training and Research. The section on the development of symptoms is a delightful breath of fresh air. The authors examine the origins of common childhood psychiatric symptoms-specifically, anxiety, depression, psychotic thinking, hyperactivity, impulsivity, inattention, violence, and neuropsychiatric symptoms. All these issues are intimately connected to research data, and although the entire book demonstrates impressive reference lists, this section may be the highlight in this aspect. The material on syndromes extends beyond the familiar common line-item delineation of diagnostic categories and includes illuminating case illustrations that help correlate the complex biopsychosocial model that this textbook admirably uses. The material in the section on treatment is eclectic and comprehensive, in that its scope ranges from pharmacokinetics to psychodynamic psychotherapy to primary prevention.

In such an overall well-done work, citing a section that "stands above" the others is difficult; however, the section on child psychiatry and allied professions seems to do that. This section discusses child psychiatry and pediatrics, child psychiatry and education, child psychiatry and law, and child and adolescent psychiatry and public health. Although many textbooks purport the absolute necessity for child psychiatry to be closely aligned with these areas, this book develops all these areas in a scholarly fashion. I recommend this publication to all medical students, residents, and fellows who have an interest in the development, diagnosis, and treatment of psychiatric illness in children and adolescents. The audience for this book, however, extends beyond students and includes practicing child psychiatrists, adult psychiatrists, pediatricians, family practitioners, allied health-care professionals (specifically, nurses, social workers, and therapists), psychologists, educators, and lawyers. From the beginning, this book never "loses" its empathetic position with its precious subject matter-i-children who are affected by their various diagnoses. This text is a seminal work in the field of child and adolescent psychiatry and can be recommended to all who have an interest and commitment to the emotional well-being of children and adolescents.
