Humoral immunodejciencies

24 June 1997 - Poster presentations

Methods of Study: peripheral blood values, crttertes of immune system, coagulation, spontaneous platelet aggregation (SPA), bactenologic study. New immunologic marker-index of allergic inflammation (IAI) was counted to reflect immune compartment interrelations as indicator of severity of BA. Computer spirometry was performed. Therapeutic programmes included corticosteroid therapy (CT). Significant correlations were revealed in whole group between forced exptratory volume (FEVI) and IAI, FEVl and SPA. IAI was the best marker of asthma severity in 1, 2 and 5 CV, SPA - in 6 CV, erythrocyte sedimentation rate - in 4 CV. Markers of phagocyttc activity were increased in 2 and depressed in 5 CV. After CT we registrated significant depression of IAI and increase of FEVl in accordance with good clinical results in whole group of BA. The best results were in 3 and 5, but more less in 6 CV. SPA and other hemostatic parameters didn’t normalized after CT.

As a resuft of the conducting investigattons the efficient diirences of the studied indices in the different forms of allergosis and secondary immunopathologic states were revealed. The changes of the immune status of persons, living in different geographical zones were came to light. It was determined the sensitized influence on the industnal discharge of the works. On the grounds of the driven data the informative indications were determined and the principle of their usage was worked out in the diagnosis and in the prognosis of the different forms of the immunopathdogy. dependent on the process of the stage.

Wide spread of immunodependent and allergy diseases in the last years is connected with influence of ecological factors and inspection of the people population by immunological methods is detemtinated the second immunodeficiency in the different percent of cases. 140 patients with chronic bronchitis, 50 patients with lymphocytic leukemia, 60 patients with lymphoma, 60 patients with Herpes -1,2 and cytomegalovirus infections were investigated immunologically (CDSC, CD2+, CD4+, CD6+, CD16+, CD21+, B-cells with surface lg, serum IgG, IgA, IgM, level of HSV-I,2 and CMV antibodies). Percent of patients with immunological changes in the peripheral blood was different and connected with level oftheimmunological system alteration. 100% of patients with lymphocytic leukemia and 50% patients with herpes viruses infections have immunological alterations of T and B-cetls, CDt6+ cells in the peripheral blood. 50% patients with chronic bronchitis have immunological changes of T-cells in the blood and 66% patients have alterations in the local immunological system of the respiratory tract. In 20% patients with chronic bronchitis and 30% patients with herpes viruses infection thyroid hypofunction was detemtinated. We propose the work classification of the second immunodeffciencies. Classification of the second immunodeficiencies: 1. Transient - prolongation 6 months and less; 2. Chronic-prolongation more than 6 months; 3. Systemic - prolongation more than 6 months, alteration more than one system or organ; 4. with alteration of neuro-endocdnological system; 5. with alteration of immunological indices in the peripheral blood; 6. with alteration of local immunological system.

The professional chemical factors in the development of the allergy and second immunodeficiencies

I.S. Dovzhansky. Institute of Rural Hygiene, Saratov. Russia The mass conducting of the immunologic investigations of the workers, exposed to the influence on the chemical agents, and the inhabitants in the unfavourable regions has the essential significance with the view to prognosticate the development of the pathologic processes. It is used the complex of the laboratory indices under the investigation of 543 people practically healthful, patients with allergosis, chronical relapse infections and oncopathology, living in the regions of the chemical works and disposed in the different geographical zones. It included the tests, descttbing the state of the cell and humoral immunity, the system of the biogene amines, adaptional mechanisms and the methods, revealing the allergic influence on the products of works. The information estimation of the using tests is carried out by the means of the mathematical calculation of the informative measures for the selection of the minimum quantity of symptoms, that are necessary for diagnosis of the pathologic states.

The aim of investigation is to study the immunological abnormalities, accompanying acute and chronic viral and bacterial infections in children; to examine the immunological status of general and local immunity in children with recurrent infections; to study the mechanism of the development of secondary immunodeffciency in children, who were influenced by an unfavourable effect of environmental hazards (radiation). The important task is decreasing recurrent infections in children with immunodeficiency. 431 children with diierent clinical forms of immunodeficiency, aged from 1 to 14 years, living on the tenttory of radioecological control and on the clean territory, were examined immunologically. The immunodeffciency was diagnosed on the basis of complex immunological signs of the function of cellular and humoral immunity and nonspecific resistance. The data of immunograms of children from the territory of radioecological control differ from standards. These immunograms are characterised by more deficiency of the general population of T-lymphocytes, suppressed functional activitv. breach of the correlation between reoutative suboooulation of T-lymphocyt& by increased of B-population of lymphocytes with dysfunction of immunoglobulin classes, by suppressed phagocytic activity of neutrophilic leukocytes and increasing circulation immune complexes. It’s an evidence of an unfavourable effect of the radiation on children’s immunity. For the first time data about spreading different forms of immunodeficiency of children from territory ot radioecological control were received. lmmunodisorders were corrected by immunotherapy.

Introduction: The Spanish Registry for Primary lmmunodeficiencfes (REDIP) was organized in 1993. hleteriats and Methods:One thousand and one hundred forty five cases of primary immunodeffciencles (PID) were registered from patients diagnosed from January 1960 to December 1996. PID diagnosis was made according to the WHO criteria. Resufts: The most frequent disorders were IgA deficiency (n = 430) and common variable immunodeflcfency (n = 222), followed by severe combined immunodeficiency (n = 63) Cl inhibitor deficiency (n = 56) X-linked agammaglobulinemia (n = 54), IgG subclass deficiency (n = 46) and chronic granulomatous disease (n = 47). Usina the official Soanish and Balearic Islands demooraohic statistics. the incidend;, per l@ live’ births and five years periods (19&&l, 1965-69’and 1990-1995) of the antibody deficiency group and other selected immunodeficiencies were calculated. Due to the peculiar geographic characteristics and health organization, Balearfc Islands PIDS incidence were used like standard value to calculate the Spain PIDS standard incidence rate (SIR) for 1990-1994 years period, this gives a result of 1917 anticipated cases vs 390 registered. Autoimmunity was reported in 49 patients (4.5%) and 14 developed cancer (1.3%). Except for complement deficiencies, male sex was predominant. In general, the time elapsed between onset of ctinical symptoms and PID diagnosis was considerable, ranging from 30 months for IgA-D to 32 months for X-LA, 74 months for IgG subclass deficiency and 132 months for CVID. Gammaglobulin replacement therapy was applied in 406 patients and bone marrow transplantation was performed in 41. Conclusions: The role of primary immunodeffciency registnes is fundamental, not only to obtain epidemiological data of these pathologies, but also and perhaps more importantly, to raise awareness within the medical profession, to facilitate information concerning new immunodeficiency diseases, ensure access to diagnostic test and up-dating of treatments.
