A 50 year old mechanic, who had a history of smoking and using vibrating tools, was referred for assessment of a possible palmar abscess. He complained for more than a year of a gradually expanding mass on the palmar surface of the right hand. The only notable abnormal sign was a positive Allen test with refill exclusively from the right ulnar artery. Duplex ultrasonography and a subsequent digital subtraction angiogram were performed, confirming the diagnosis of an occluded radial artery and an ulnar artery aneurysm. Six months after diagnosis, there is no increase in the diameter of the aneurysm.
