It is with great pleasure that we introduce this symposium on gynecologic pathology in honor of Dr Robert E. Scully on the occasion of his 70th birthday on August 3 1, 199 1. Dr Scully has been a member of the Editorial Board or Associate Editor of Human Pathology since its inception., and contributed an influential review on ovarian tumors to the vety first issue of the Journal. He also was co-editor of the last symposium on gynecologic pathology that appeared in the Journal in 1977. It is therefore appropriate that Human Pathology selve as the forum for this sytnposium and we are grateful to the Editor, Dr Gorstein, and the Associate Editor for Symposia, Dr Pascal, for facilitating its publication. We would like to use this opportunity to acknowledge our own debt and that of pathologists worldwide to Dr Scully for his immense contributions to surgical pathology, particularly gynecologic and testicular pathology. Before commenting on the symposium itself, we believe that a brief review of Dr Scully’s career and contributions is appropriate, especially for those who know Dr Scully only through his writings and lectures. At the same time we hope to indicate why those of us who have been fortunate to have worked with him have found the association so rewarding. We are most grateful to Dr Aleksander Talerman, Professor of Pathology at Thomas Jefferson University Medical School and current President of the International Society of Gynecological Pathologists, and Dr Arthur Herbst, Joseph Bolivar DeLee Distinguished Service Professor and Chairman, Department of Obstetrics and Gynecology, University of Chicago, for writing personal appreciations of Dr Scully and elaborating on some other facets of his career. Dr Herbst was on the gynecologic staff of the Massachusetts General Hospital (MGH) for many years, and during this time worked closely with Dr Scully on studies, which are still ongoing, on the abnormalities of the female genital tract associated with intrauterine exposure to diethylstilhestrol. After graduation from Harvard Medical School in 1944, Dr Scully embarked on a 3-year internship and residency in pathology at the Peter Bent Brigham Hospital and Children’s Hospital in Boston. Parham, beginning an interest in the testis that has continued over the years. As a result, although best known as a gynecologic pathologist, Dr Scully is also recognized worldwide for his expertise in testicular tumors and the evaluation of testicular biopsies. He has accumulated an extensive collection of unusual testicular neoplasms, resulting in his defining a number of new subtypes of testicular tumors and co-authorship of a recent text on this topic. Dr Scully’s interest in gynecologic pathology postdated his interest in testicular pathology, but by only a short period of time, as his residency at the Peter Bent Brigham Hospital was followed by a year as resident at the Free Hospital for Women and Boston Lying-in Hospital, where the late Dr Arthur T. Hertig was the Chief of Pathology. Not surprisingly, Dr Scully’s experience at these institutions, if only for a relatively brief period of time, set in motion a life-long study of gynecologic pathology. After 1 further year as a resident at the Pondville Hospital, Dr Scully was recruited by Dr Tracy B. Mallory to join the staff of the MGH in 1950, where he has remained with the exception of 2 years spent in the Armed Services during the Korean War. When Dr Scully joined the staff of the MGH, he was one of only four full-time staff pathologists working under Dr Mallory’s direction. The others were Dr Benjamin Castleman, Dr Hank Fennell, and Dr Austin L. Vickery, Jr. Dr Ronald Sniffen additionally helped out on a part-time basis. Dr Vickery, Dr Scully’s longtime friend and colleague, has had an office beside Dr Scully’s for most of the last 40 years. Shortly after Dr Scully’s arrival at the MGH, the die was cast in assuring that gynecologic pathology would be one of Dr Scully’s major areas of special interest. Dr John McLean Morris, a g-ynecologist on the MGH staff and subsequently Chief of Gynecology at Yale University, had undertaken to write a book on the endocrine pathology of the ovary and needed a pathologist to assist him with this endeavor. His collaboration with Dr Scully resulted in their publication, Endocrine Pathology of the Ovary, in 1958. The interest in functioning tumors of the ovary expressed in this publication resulted in Dr Scully, from a relatively early stage in his career, being sent examples of many interesting ovarian and other gynecologic tumors. Dr Scully’s careful observations and his access to an increasingly large collection of unusual cases over the years has led to a long list of original contributions, most of which will be familiar to the reader (see the accompanying bibliography). When the link between intrauterine exposure to diethylstilbestrol and clear cell adenocarcinema of the female genital tract first became apparent in the early 1970s Dr Scully, together with others at the MGH, was responsible for a series of papers documenting the nature of this association and the various neoplastic and nonneoplastic abnormalities encountered in these patients. With respect to his activities at the MGH, Dr Scully undertook regular turns signing out general surgical pathology until relatively recently, when the volume of his consultation practice became such that some routine duties had to be discontinued. He still takes his regular 738

turn in frozen section coverage. Although known to the outside world primarily for his expertise in gynecologic and testicular pathology, generations of residents and staff pathologists at the MGH are well aware of Dr Scully’s superb skills in many areas of surgical pathology, as he had the good fortune to train at a time that preceded the current trend toward subspecialization. This has made him, throughout his career and to this day, an invaluable consultative resource in a wide range of problem areas. The alacrity and speed with which he can make a diagnosis on a very challenging case has impressed generations of residents and staff. Most readers will not be aware that for many years Dr Scully maintained a strong interest in cardiac pathology, which he taught at Harvard Medical School. An additional major responsibility of Dr Scully’s over the years has been a very active participation in the case record publications of the MGH, initially as the pathologist on numerous cases beginning in 195 1, then as Associate Editor, and finally as Editor since 1974. In addition to the selection of the case for each week’s presentation, the last role requires the editing of each case prior to publication, a major time commitment that is tantamount to being responsible for a case report every week. This obligation, on top of his others, means Dr Scully has to be adept at taking advantage of every opportunity to do some editing, as was proven to one of us some years ago when waiting with him for luggage at Los Angeles airport. In the midst of proverbial “mass confusion,” Dr Scully calmly reached into his briefcase, pulled out the familiar manilla folder containing one of the case records awaiting his editing, and set to work seemingly oblivious to his surroundings! Dr Scully’s responsibility for the case records allows full range for his legendary editorial pen, which all who have written papers with him have come to know with a mixture of respect and trepidation. The latter word reflects the angst we have all felt when the draft of a manuscript thought very creditable by the submitter has been returned with a vast number of comments, including grammatical and editorial changes, in Dr Scully’s instantly recognizable neat writing. The initial despair is very quickly replaced by a realization that the polish that Dr Scully provides in both content and presentation invariably results in the final paper being a great improvement on its forerunners. Dr Scully’s contributions have been recognized widely over the years in many ways. He has held the rank of Professor of Pathology at the MGH and Harvard Medical School for many years. Each year he travels extensively in the United States and undertakes frequent trips to foreign countries giving lectures and slide seminars. His careful preparation and attention to detail are highlighted by the fact that he still makes a list of all the slides to be shown in a talk, even one he has given numerous times, with an indication as to the points to be emphasized while they are being shown. Among his many honors, perhaps four of the most noteworthy are the Fred W. Stewart Award of the Memorial Sloan-Kettering Cancer Center in 1980, the Arthur Purdy Stout Award and Lectureship of the American Society of Clinical Pathologists (ASCP) in 1983, the Distinguished

Pathology Educator Award honoring H. P. Smith, also of the ASCP, in 199 1, and the Maude Abbott Lecture of the US,‘Canadian Academy of Pathology, to be given in March 1992. If the reader wonders how Dr Scully has found the time to accomplish all he has, it will not be a surprise to learn that the term “hard worker” in no way does justice to the hours he has worked over the years, almost always being the first in the department every day (including Saturdays, Sundays, and holidays). The majority of hlis time in recent years has been spent studying and dictating responses to his very large consultation practice, his mailbox invariably being tilled with containers from all over the United States and other parts of the world everv morning. Each case is handled individually by Dr Scully, unless he is out of town, and is given a number bearing the well-known prefix “SCS” (Scully consu11 ation series). Fortunately for those of us who have been able to utilize Dr Scully’s wonderful collection for publications, he has always been meticulous in coding and cataloguing his cases, which now number over 22,000, a great percentage of them of an unusual nature. This collection serves as a superb teaching resource for the many visitors who spend time looking at the slides and reading Dr Scully’s careful letters of reply that are oFten embellished with appropriate references and comments on therapy. Dr Scully conducts the daily departmental surgical pathology conference every Monday, based on his consultation cases from the previous week. and the range of unusual cases shown and discussed by him never ceases to amaze visitors to the department. When planning this symposium we had some initial indecision as to the most appropriate manner in which to select the contributors. There are obviously many pathologis.ts who could have written outstanding reviews on the ar’eas l.overed herein. However, we ultimately thought that the most appropriate contributors would be those who have had a personal association of significant duration with Dr Scully. All the contributors have spent at least 1 year, and in most instances longer periods, as fellows with Dr Scully. We hope we have all learned enough through his guidance to make the accompanying reviews of a standard that Dr Scully would approve, even in the absence this time of his editorial hand. Rigid space constraints have precluded any efforts to make these reviews encyclopedic, profusely illustrated, or exhaustively referenced. Rather, they are an attempt to highlight and summarize some of the important developments in various areas of gynecologic pathology during recent years. In closing, we are sure that the readers of the Journal will jolm the contributors to this symposium in wishing Dr Scully continued good health so that his unique contributions to pathology will continue for many years to come.
