D E N TA L P R A C T I C E Articles accepted for publication are subject to editorial revision. Statements and opinions expressed in the articles and communications herein are those of the author(s) and not necessarily those of the Editor(s) or publisher, and the Editor(s) and publisher disclaim any responsibility or liability for such material. Neither the Editor(s) nor the publisher guarantees, warrants, or endorses any product or service advertised in this publication. Neither do they guarantee any claim made by the manufacturer of such product or service. Should there be a financial association between one or more of the authors and a commercial company that makes a product that figures prominently in the article, the Editors stipulate that the authors clearly state such an association. Authors are requested to furnish a clear written statement to the Editor-in-Chief concerning any real or perceived conflicts of interest. Should a significant conflict of interest be present, the Editors reserve the right to reject the article on that basis. Only original manuscripts will be accepted. Manuscripts are generally not returned to authors. Original illustrations are returned only if requested by the author.
M ANUSCRIPTS The Journal of Evidence-Based Dental Practice has two primary goals: • To improve patients’ outcomes and health • To facilitate decision-making that results in better patient outcomes, enhanced research planning, better products, and improved policy development Manuscripts that review or address the Journal’s goals are welcome. Current status of an evidence-based topic, systematic reviews, and articles on diagnosis, or treatment are encouraged. Emphasis for acceptance includes a clear statement about how the manuscript content addresses the Journal’s goals. Clarity of presentation is very important.
Original articles are invited that: • Demonstrate the clinical application of an evidence-based approach • Provide specific examples of how an evidence-based approach has been integrated into clinical practice • Present new techniques for teaching application of the evidence-based approach • Present original research providing compelling evidence that may influence the practice of dentistry • Facilitate decision–making that results in better patient outcomes, enhanced research planning, better products, or improved policy development. Editorials are also invited.

Copyright statement. The specified copyright statement that appears at the end of the Information for Authors must be completed, signed by all authors, and submitted with the manuscript. If not completed in full, it will be returned to the author for completion. P REPARATION
OF MANUSCRIPTS
Manuscripts should be typewritten on one side of the paper only, with double-spacing and liberal margins. The original and one copy should be submitted. Most word-processing programs are acceptable, although IBM-WordPerfect or Macintosh–Microsoft Word is preferred. Disks should be labeled with the computer type (IBM or Macintosh), word-processing program, title of paper, and first author’s name. The word processor’s default typeface and type size should be used. Text formatting capabilities should not be used. Number pages consecutively beginning with the title page. Title page. The title page of the original manuscript should include the title of the article, the full name of the author(s), academic degrees, positions, and institutional affiliations. Listed authors should include only those individuals who have made a significant creative contribution. The corresponding author’s address, business and home telephone numbers, fax number, and e-mail address should be given. Authorship. All persons who are identified as authors should make substantial contribution to the manuscript through significantly contributing to the conception, design, analysis or interpretation of data, drafting or significantly revising the manuscript, and providing final approval of the manuscript. All three of these conditions must be met by each author. Persons who contribute to the effort in supportive roles should not be included as authors; rather, they should be acknowledged at the end of the paper. References. References should be cited selectively. Personal communications and unpublished data are not to be cited as references; instead, they should be cited in parentheses at the appropriate place in the text. Make sure all references have been verified and are cited consecutively in the text by superscript numbers. Reference list format should conform to that set forth in “Uniform Requirements for Manuscripts Submitted to Biomedical Journals” (Ann Intern Med 1997;126: 36-47). A copy of these Requirements may be obtained from the Editors or viewed/printed online at www.icmje.org. References to articles in press must include authors’ surnames and initials, title of article, and name of journal. The reference list should be typed double-spaced on a separate page and numbered in order as the reference citations appear in the text. For journal citations, include surnames and initials of authors, complete title of article, name of journal (abbreviated according to the Cumulated Index Medicus), year of publication, volume number, and inclusive page numbers. For book citations, surnames and initials of authors, chapter title (if applicable), editors’ surnames and initials, book title, volume number (if applicable), edition number (if applicable), city and full name of publisher, year of publication, and inclusive page numbers of citation. EXAMPLES (if six or fewer authors, list all; if seven or more, list first six and add “et al”): Format for periodical references: Pullon PA, McGivney J. Computer utilization in an oral biopsy service. Int J Oral Surg 1977;6:251-5.

Format for book references: Seakins J, Saunders R, editors. Treatment of inborn errors of metabolism. London: Churchill Livingstone; 1973. p. 51-6. Format for chapter references: Hudson FB, Hawcroft J. Duration of treatment in phenylketonuria. In: Seakins J, Saunders R, editors. Treatment of inborn errors of metabolism. London: Churchill Livingstone; 1973. p. 51-6. Illustrations. Illustrations must be in black and white. Illustrations (two sets of glossy prints) accompanying manuscripts should be numbered, provided with suitable legends, and marked lightly on the back with the article title and an arrow to indicate the top edge. Drawings or graphs should be prepared in black India ink. Typewritten or freehand lettering on illustrations is not acceptable. All lettering must be done professionally, and letters should be in proportion to the drawings or photographs on which they appear. Do not send original artwork or x-ray films. Because good black-and-white contrast is essential for the best reproduction, only glossy print photographs (no smaller than 3 × 4 inches and no larger than 5 × 7 inches) are acceptable. Consistency in size of illustrations within an article is strongly preferred, and special instructions concerning sizing should be specifically noted. Roentgenograms should be supplied as prints showing bone areas as white. Illustrations will be returned only if requested by the author. The size of the illustration should be as close as possible to the standard size of illustration in this Journal. Electronic illustration submission. Figures may be submitted in electronic format provided that high-quality digital or glossy prints are also included. All images should be at least 5 inches wide. Images should be provided in EPS or TIF format on Zip disk, CD, floppy, Jaz, or 3.5 MO. Both Macintosh and PC are acceptable. Graphics software such as Photoshop and Illustrator (but not presentation software such as PowerPoint, CorelDraw, and Harvard Graphics) should be used in the creation of the art. Gray-scale images should be at least 300 DPI and be accompanied by a high-quality digital or glossy proof. Combinations of gray-scale and line art should be at least 1200 DPI and be accompanied by a high-quality digital or glossy proof. Line art (black-and-white) should be at least 1200 DPI and be accompanied by a high-quality digital or glossy proof. Please include hardware and software information (in addition to file names) with the disk. Legends to illustrations. Each illustration must be accompanied by a legend. These should be typed double-spaced on a separate sheet of paper. If an illustration has been taken from published material, the legend must give full credit to the original source. Tables. The tables should be typewritten double-spaced, including column heads, data, and footnotes, and submitted on separate sheets. Tables should be self-explanatory and should supplement, not duplicate, the text. A concise title should be supplied for each table. All columns should carry concise headings describing the data therein. Type all footnotes immediately below the table and define abbreviations. If a table or any data therein have been previously published, a footnote to the table must give full credit to the original source. Permissions. Direct quotations, tables, or illustrations that have appeared in copyrighted material must be accompanied by written permission for their use from the copyright owner

and original author, along with complete information as to source. Photographs of identifiable persons must be accompanied by signed releases showing informed consent. Articles appear in both the print and the online versions of the journal, and wording should specify permission in all forms and media. Failure to obtain electronic permission rights may result in the images not appearing in the online version. Copyright statement. The following copyright statement must be signed by ALL authors: “The undersigned author(s) transfers all copyright ownership of the manuscript entitled to Mosby, Inc, in the event the work is published. The undersigned author(s) warrants that the article is original; is not under consideration by another publication; and its essential substance, tables, or figures have not been previously published.
