19-year-old white man was referred to the oral surgery clinic at the University Hospital for evaluation of left facial swelling and multiple jaw lesions. The patient was admitted for treatment of an infection and, after a comprehensive evaluation, a diagnosis of basal cell nevoid syndrome was made. Among the other findings were a broad and flat nasal bridge, hypertelorism, hand pitting, bifid uvula, vertebral anomalies, calcification of the falx cerebri, and bridging of the sella turcica. A panoramic radiograph revealed radiolucent lesions of the left posterior maxilla, the left anterior maxilla, the left body and symphysis of the mandible, and the right ramus and body of the mandible.

After resolution of the infection involving a lesion in the left mandible, all four radiolucent lesions were explored and curetted in conjunction with surgical removal of the unerupted teeth. Histologic study revealed that these lesions were odontogenic keratocysts.
