Peer review report 1 on “Importance of latency and amplitude values of recurrent laryngeal nerve during thyroidectomy in diabetic patients” 1. Original submission 1.1. Recommendation Accept. 1.2. Comments to the author This is a highly interesting paper looking at neural monitoring waveform data in diabetics as compared to nondiabetics. It is unique in that this data set has not been previously reported on in the nerve monitoring literature. It is certainly intuitive that the waveform characteristics in diabetic nerves may be different and this is in fact what these researchers found in a large group of patients 26% of whom had

diabetes. They found a significant decreased amplitude and increased latency as one would expect in diabetic nerves. This paper is wellwritten and has an especially expansive and appropriate literature review. It is of note that the current understanding of neural injury identified intraoperatively is a finding (termed combined event) that couples amplitude reduction with concordant latency increase.
