1. Original submission 1.1. Recommendation Accept. 1.2. Comments to the author It is a very interesting case series on the surgical management of selected locally invasive (vertebral) lung cancer entailing a lobectomy and vertebrectomy. The 5yr survival of 71% was impressive although there was only 50% disease-free survival. This may indicate an important role for neoadjuvant chemo/radiotherapy which was given to 2 out of the 8 patients. These patients did not have

local recurrence or distant metastases. Was there any reason why all the patients did not receive neoadjuvant therapy? A principle in surgical oncology is that the response to neoadjuvant chemo/ radiotherapy may predict the prognosis following surgery. The multidisciplinary approach was well appreciated.
