Fudan University Shanghai Cancer Center, Dept. of Urology, Shanghai, China INTRODUCTION & OBJECTIVES: A novel nomogram using RENAL nephrometry score was developed byKutikov and colleagues for predicting high grade renal cell carcinoma and showed good performance in internal evaluation. The current study was undertaken to externally validate the prediction model. MATERIAL & METHODS: We identified a cohort of 391 Chinese patients with surgically resected renal cell carcinoma at our institution from 2008 to 2011. Fuhrman grade was reviewed by one experienced genitourinary pathologist and radiologic images were independently assessed by two senior urologists. Using a two-tiered system, highgrade disease was defined as Fuhrman grade III/IV. The statistical performance of the prediction model was evaluated by discrimination, calibration and clinical usefulness. RESULTS: Of 391 patients, 45.5% were assigned to have high grade tumors. External validation of the nomogram demonstrated an area under curve of 0.73. The calibration plot showed the predicted probabilities of high grade disease were comparable concordance with the observed frequency. Decision curve analysis showed the prediction model provided superior net benefit and reduction with the probability threshold > 20%. CONCLUSIONS: We confirm the predictive value of the nomogram using RENAL nephrometry score for identifying high grade renal cell carcinoma in an independent cohort. Further research will be required to evaluate its performance with head-to-head comparison with renal biopsy results.
