A false passage was accidentally created in the anterior uterine wall during the course of carbon dioxide hysteroscopy being performed in a case of infertility. false passage was immediately recognized by the characteristic appearance of the circular myometrial fibers and nonvisualization of any endometrial gland opening. While the hysteroscope was being withdrawn, 2 distinct openings were seen located above and below a horizontal band. On closer inspection, the upper opening is seen leading to the false cavity, whereas the lower opening led to the actual uterine cavity (Figure 1). The hysteroscope was reinserted through the lower opening, and the diagnostic hysteroscopy was successfully concluded (Figure 2). The postoperative period was uneventful.
