A Modified Hegar Needle Holder

The Hegar and the Hegar-modified needle holders are largely preferred by the majority of surgeons. Their handling is a matter of personal choice. Benjamin [I], however, emphasizes the advantages of the palm grip over the most commonly used thumb-finger grip. He devised a ringless needle holder for proper training of the palm grip. We were unable to do satisfactory work with the Hegar needle holder whenever we tried the palm grip as suggested by Benjamin. Its use was simplified, however, by inserting a spring similar to that of the Mathieu needle holder between the shafts of the conventional Hegar needle holder. (Figure 1.) Its jaws

can be opened by simply pressing the shaft with the thumb in a delicate but firm movement. (Figure 2.) This modification makes the Hegar needle holder a more versatile instrument to be used easily in either the thumb-finger grip or the palm grip. In our experience it has proved to be particularly valuable in cardiovascular surgery. Summary

A spring was inserted between the shafts of a conventional Hegar needle holder. This facilitates the palm grip handling and does not interfere with the thumb-finger grip, resulting in a more versatile instrument. Reference
