Department of Endocrinology and Metabolism, Nippon Medical School, Tokyo; 2Internal Medicine, Sendai Red Cross Hospital, Sendai, Miyagi Objectives: Chylomicron and its remnant have a characteristic apolipoproteinB48 (apoB48), and we previously demonstrated that apoB48 level is strongly associated with atherosclerosis. Metabolic syndrome (MeS) and diabetes mellitus (DM) are risk factors for atherosclerosis, and common atherogenic lipoproteins between them are remnants. Remnants are heterogenous and their quantitative determination is difficult. Recently, the ELISA for apoB48 level has been developed. Our aim was to investigate whether there was the difference in apoB48 levels among the subjects of normal, metabolic syndrome, and DM. Methods: Subjects (n=299, male 231, female 86, age 46 ± 10 years) [mean ± SD] who got health examination at Sendai Red cross Hospital, northern part of Japan, were classified as normal (n=251), MeS (n=19), and DM (n=23). The criteria for DM were defined as fasting plasma glucose 126m/dl. Results: Serum apoB48 levels of normal, MeS, and DM were 3.8 ± 5.8, 8.8 ± 7.7, 7.4 ± 5.5mg/ml, respectively. (MeS versus normal p<0.05, DM versus normal p<0.05) Conclusions: Serum levels of apoB48 were elevated in MeS and DM. These results suggested that chylomicron metabolism will be impaired in such metabolic diseases.
