Dental schools that have approval, conditional approval, or provisional approval status are listed below. All programs have approval status ex­ cept those designated with a cross (+) or an asterisk (*). A cross (+) in­ dicates that the program has condi­ tional approval status. An asterisk (*) indicates that the program has provi­ sional approval status. The year fol­ lowing the name of each institution indicates the next regularly sched­ uled evaluation visit on a seven-year review cycle. It does not preclude the Commission from recommending that an evaluation be conducted prior to the designated year. Information about new programs is included at the end of the list. Definitions of ac­ creditation classifications appear in the addendum to this listing.

Addendum The following accreditation classifi­ cations have been approved by the Commission:

Preliminary Provisional Ap­ proval. An accreditation classifica­ tion used by the Commission on Ac­ creditation to grant initial accredita­ tion to an educational program based on the review of a Commission appli­ cation and survey manual. This clas­ sification is granted to assure the educational institution and other agencies that the program is develop­ ing or has developed according to the guidelines established by the Com­ mission. This classification provides assurance of candidate eligibility for certification programs.

Accreditation Eligible. An ac­ creditation classification granted to any program which is in the plan­ ning or early stages of development or an intermediate stage of program implementation. This accreditation classification provides evidence to the educational institution, a licens­ ing body, the federal government, or other granting agency that, at the time of evaluation, the developing education program appears to have the potential for meeting the stan­ dards set forth in the requirements for an accredited educational pro­ gram for the specific occupational area. The classification “accredita­ tion eligible” must be based on a site evaluation visit. Approval. An accreditation clas­ sification granted to an educational program indicating that the program in general achieves or exceeds the basic requirements for accreditation. This accreditation classification specifies that the program has no serious deficiencies or weaknesses; however, recommendations or sug­ gestions relating to program en­ hancement are generally included in the evaluation report. Conditional approval. An accredi­ tation classification granted to an educational program indicating that specific deficiencies or weaknesses exist in one or more basic areas of the program. The deficiencies or weak­

nesses are considered of such a nat­ ure that they can be corrected in a reasonable length of time which is ordinarily defined as a period not to exceed two years.* This accredita­ tion classification is considered adequate to meet the eligibility re­ quirements for licensure and certify­ ing board examination. Provisional approval. An accredi­ tation classification granted to an educational program indicating that the program has a number of major deficiencies or weaknesses in one or more specific areas. This accredita­ tion classification signifies the seri­ ousness of the deficiencies or weak­ nesses but is considered adequate to meet the eligibility requirements for licensure and certifying board examination. The deficiencies or weaknesses are considered to be of such magnitude that, if not corrected, withdrawal of the program’s accredi­ tation will result. Evidence of signif­ icant progress must be demonstrated within one year. *Prior to termination of this pre­ scribed period, the Commission will either review a progress report, re­ quest that an institutional repre­ sentative appear before the Commis­ sion, or conduct another site visit to determine if the deficiencies or weaknesses have been satisfactorily corrected.
