A 65-year-old man presented with atypical subcostal pain on the right side. Abdominal ultrasound revealed a spherical lesion near the upper pole of the right kidney with turbulent high-velocity flow spectrum. Additional computed tomography angiography scan confirmed a 6-cm right renal artery aneurysm with parietal thrombus. Endovascular treatment was conducted by a left brachial artery approach with exclusion of the aneurysm by placement of a covered stent (Viabahn 7 50 mm; W.L Gore & Associates, Livingston, UK). Completion angiography confirmed aneurysm exclusion with no renal loss, and the patient was discharged on dual antiplatelet therapy (aspirin 100 mg, clopidogrel 75 mg).
