T he chapter on sali­ cylates, although inform ative, covers a great deal m ore than the antihem o­ static properties o f the drugs and may seem too lengthy if one is p ri­ m arily interested in blood coagula­ tion. The m aterial presented does pre­ suppose some background in the field o f hem ostasis, especially since Q uick’s choice of the “best” tests for hem ostatic disorders may not agree w ith those of other investigators in the field. Nonetheless, the book is recom m ended as a valuable supple­ m ent to other texts discussing bleed­ ing disorders to those dentists who have a keen interest in hemostasis o r who, by the nature of their p rac­ tice, encounter bleeding problem s frequently. T his m onograph by A rm and Q uick— a recognized authority and im por­ tan t contributor in the field of bleed­ ing problem s— is both inform ative and well w ritten. It m ay be read in a single concentrated evening o f study. A n interesting but concise history of the developm ent o f the field o f blood coagulation serves as an appropriate beginning and the book then p ro ­ gresses into the pathogenesis and clin­ ical diagnosis o f both com m on and quite uncom m on causes o f abnorm al bleeding. T here is an excellent dis­ cussion o f the M inot-von W illebrand syndrom e w hich has heretofore re­ ceived too little attention as a signifi­ ca n t cause o f bleeding. A s the tim e spent in basic science courses currently seems inversely proportional to the size o f definitive texts, the “concise” text and outline are both receiving serious attention by instructor and student alike as a m eans o f conveying a reasonable vol­ um e o f inform ation in a systematic way. G errit Bevelander’s Essentials o f Histology is representative o f a short text. Being approxim ately a third the length o f definitive texts, it nevertheless deals com prehensively w ith the subject m atter o f histology including a section on the structure o f the brain and special sense organs. B oth the au th o r and publisher are to be congratulated for realizing the ab ­ solute im portance of illustrative m a­ terial in a histology text; the sixth edition contains 73 m ore figures, mostly electrom icrographs, than the previous edition. However, reproduc­ tion o f m any light and electrom icro­

graphs leaves considerable room for im provem ent. Labels on m any elec­ trom icrographs are exceedingly dif­ ficult to see (for instance Fig 13-9 and 14-7). T he subject m atter is dealt with evenly and the student will gen­ erally encounter a thorough treat­ m ent o f structures. T he extensive index is both a definite aid and a rare finding in short texts o f this sort. It is hoped th at elim ination o f older term inology (reticulocyte, sarcostyle, interprism atic substance) will be forthcom ing. Also the consistent use o f one term such as “odontoblas­ tic processes” in place o f “dentinal fibrils” and “T om e’s dentinal p ro ­ cesses” (p 158-159) although result­ ing in stylistic tedium w ould im ­ measurably clarify the p o in t being made. Im proved labeling (why isn’t the intercalated duct in Fig 15-3 la­ beled?), better correlation between text and figure m aterial (why is the oral m ucous m em brane shown in Fig 13-17?), and tighter editing (mast cells do n ot produce antibodies, p 37), w ould im prove this text. Essen­ tials o f Histology does serve as a well-illustrated, concise exposition o f tissue and organ architecture and, as such, fulfills both its stated purpose and a cu rren t need. Donald K . T his booklet represents a report p re ­ sented to the 21st F rench Congress o f Stom atology. It is an exhaustive an d accurate study on oral and cuta­ neous lichen planus. It contains a good historical review that shows the evolution in term inology and defini­ tion o f lichen planus. Clinical description, sex, race, and age distribution o f the condition are com pletely outlined. Com parative charts from different authors dealing w ith these aspects are also presented. H istopathology is extensively cov­ ered. A classification o f lichen planus is presented. Besides the four basic recognized forms, the authors divide the m alady according to location and secondary com plications, that is, gingiva, palate, hem orrhagic, pig­ m ented, and so forth. It seems to the present reviewers that this subdivi­ sion represents unnecessary com plex­ ityT he possible association o f lichen planus and squamous-cell carcinom a o f the oral cavity is also dealt with at length. T he figures presented by the authors seem higher than those p re­ viously reported. Etiologic theories are discussed ob­ jectively. T h e last two chapters, co n ­ cerned w ith diagnostic procedures and treatm ent are excellent. A com ­ prehensive survey o f the literature is reflected by 615 references. P hoto­ graphs, especially color plates, illus­ trating the different forms o f lichen planus w ith extrem e accuracy, are of superb quality. T h e preface is cosigned by P. As the title indicates, this book de­ scribes th e m orphology o f the face during the em bryonic period or first 12 weeks o f gestation. T he first ch ap ­ ter studies the face during the fourth week; the next four chapters take up the fifth, sixth, seventh, and eighth weeks, and the final two chapters are devoted to the ninth through the 12th weeks. In general, descriptions are lim it­ ed to the developm ental m orphology of the face an d oral tissues. T here is little consideration o f skeletal, blood, vascular, neural, and m uscular de­ velopm ent. T he author has used text­ books o f em bryology and has not in ­ cluded curren t research papers on these subjects. H e has included an ex­ cellent series o f photographs o f cleft lip and cleft palate that enhance the description o f norm al development. James K . A very

This conference m ight have p ro ­ duced inform ation of greater value relative to oral cancer had several highly qualified dentists been in ­ cluded. T he conference, “Oral C are for O ral C ancer Patients,” held in Ju n e 1968, produced a w ealth o f upto-date inform ation on this p articu ­ lar subject. A n equal num ber of den­ tists and physicians participated so a m axim um o f well-balanced inform a­ tion could be obtained. M uch has happened in dental p ublic health in the intervening years, b ut the “principles,” if they truly are principles, rem ain unchanged. T he second edition has an additional 49 pages o f text although th e num ber o f figures rem ains at 73 and th e n um ­ ber o f tables has been reduced to 47. T h e references have increased con­ siderably; six pages have been added. In it D unning says, “ It is doubtful if any period has seen as rapid an advance in the field o f den­ tal public health as has that since the spring o f 1961 when the first edition o f this book went to press Revision o f the text thus became an urgent and a m ajor undertaking. T he chapters “Surveying and E valu­ ation,” “W ater F luoridation,” “O r­ ganization o f D ental C are,” and “P ay­ m ent for D ental C a re” have been largely rew ritten. “State, N ational, and International P rogram s,” a chap­ ter in the original text, has been sub­ divided into three chapters to deal separately w ith each o f these expand­ ing fields. N ew m aterial has found its way into m ost o f the other chap­ ters. References and suggested read­ ing m aterial, a valuable asset, have been updated. T he book presents an accurate picture o f things as they are today. In addition to providing the read­ er w ith the facts o f public health den­ tistry, the book clearly identifies the issues currently facing the profession. Included are segments on socialized m edicine, expanded duties of auxil­ iaries, the G erm an system o f dental education and care, th e N ew Z ea­ land dental-nurse plan, M edicaid, the “rig h t” to health care, and so forth. D octor D unning editorializes on m any o f these and other issues, and in an effort to be fair to the reader, he does not m ask his personal com ­ m ents as “facts.” T rends are present­ ed, examined, and com m ented on. It should be especially helpful for those in the profession who are affected by changing public policy in the de­ livery o f health care. T his particular segment, the private practitioner, fo r­ m erly rem oved and isolated from dental public health, w ould do well to look into this book carefully. It m ight help him decide about many issues that have becom e real and will require action. D unning’s com m ents m ight raise som e eyebrows, especially those of younger practitioners and the m ore activistic dental students. The best first steps usually involve asurvey and its evaluation. A sharp line m ust al­ so be draw n between com m unity p re­ vention o f disease and the treatm ent of disease.” Some m ight question the presence o f “sharp lines” in view of M edicaid and other governm ent sponsored program s, and the idea that these “sharp lines m u s t . . . be draw n” m ight likewise raise some doubts. D unning goes on to say, “Such a division o f labor protects the doctor-patient relationship for a m ajority o f the population in any com m unity, keeps the cost down o f governm ent, and m akes best use of m anpow er resources already in exist­ ence.” T he phrase “m akes the best use o f m anpow er resources” m ight excite a considerable segment o f the profession. Such a statem ent as “the fact that th ird party adm inistration of health care increases rather than reduces its cost” could also be ques­ tioned. These opinions, w hich are not new, have recently been debated. T he author engages in some aggres­ sive attacks o f his own. Particularly interesting is his opinion o f the in ­ crem ental care concept— long a standby o f public health dentists. N o area o f public health p ractice es­ capes his penetrating analysis. It is

This detailed and practical m anual thoroughly covers the intravenous route for pain and anxiety control in dentistry. A veritable bible o f m ulti­ ple techniques is presented, together w ith sections on history and develop­ m ent o f intravenous sedation and an ­ esthesia, pharm acology, clinical ap­ plications, introduction for the nov­ ice, equipm ent, em ergencies, patient m anagem ent, venipuncture, m outh packing, and philosophy o f contem ­ porary dental anesthesia. Illustrations

are num erous and well executed. As this book says, there has not been a single instance in Britain in w hich a verdict o f negligence has been returned as a result o f an anes­ thetic m ortality, in contrast to the U n ited States w here legal judgm ents are com m on, and frequently unjusti­ fied. N o doubt this distinct differ­ ence in m alpractice experience ac­ counts in some degree for the failure to em phasize a basic pretreatm ent m edical history and for the com par­ atively casual approach to laryngospasm, vom iting, and cardiopulm o­ nary arrest in the text. T he S A A D Levels o f Anaesthesia presents far m ore clearly than G uedel stages the sequential levels o f light to m oderate sedation (SAAD am nesia), deep sedation (SAAD anal­ gesia), and three levels o f ultralight general anesthesia. T he SAA D levels are well illustrated and m erit adop­ tion as a standard reference. They correspond in a descending fashion from G uedel first stage through G uedel third stage, first plane. P ro ­ tective reflexes are abolished at SA A D anesthesia level second stage and are depressed from deep seda­ tion (SAAD analgesia) downward. B ritain is far ahead of the U nited States in undergraduate teaching of intravenous psychosedation, w ith seven schools of a total o f 17 includ­ ing the subject as an elective, and w ith four schools planning to follow suit in the near future. M ore than 50% o f British dentists use the inhala­ tion route for sedation or general anesthesia in practice, and thus have a background for continuing educa­ tion in the intravenous route. A m eri­ can dentists w ithout general anesthe­ sia training should not use deep seda­ tion (SAAD analgesia) o r ultralight general anesthesia because o f airway control, but should lim it themselves to diazepam sedation, the Jorgensen technique, or equivalents in w hich the book abounds; all o f these tech­ niques produce dependable light to m oderate sedation in conjunction w ith local anesthesia. I recom m end this well-written text to readers o f t h e j o u r n a l for its general background value, rather than for any pertinence to dentistry per se. T he book does a good jo b in fitting hum an biology into the p er­ spective o f biological principles gen­ erally. M an is lim ned as a generalized and specialized anim al in the total organic realm. T here are three parts to this book: “Background,” “H um an P aleontolo­ gy,” and “H um an H eterography.” T he first p art gives a short historical sum m ary o f physical anthropology, as well as a succinct outline o f m ain classes w ithin all form s in nature. T here is a superb discussion o f genet­ ics, ranging from M endelian genetics through m olecular and biochemical genetics. T h e D N A -R N A discussion is exceptionally well done. T here is a good chapter on population genet­ ics w ith em phasis on m icroevolution. T he second p a rt presents the p rin ­ ciples o f organic evolution, prim ate evolution, and the evidence o f h u ­ m an evolution in term s of hom inoid and hom inid fossils. T he latter are fitted into the fram ew ork o f geologi­ cal and archaeological time.
