Type of Book: A co mpact paperback (easily read in one or two sittings) written by authors who have extensive experience in directing a large outpatient pulmonary function laboratory. Scope of Book: Th is well-illustrated, practical guidebook for clinicians focuses on the indications for various pulmonary function tests and the interpre tation of the test results. Contents: The first 100 pages of the book are divided into 13 short chapters that pro vide overvi ews of each of the most commonly performed tests of pulmonary function, including spirometry. static lung volumes, diffusing capacity. bronch ial challenge. and cardiopulmonary exercise testing. Almost every other page includ es a line drawing o r a table. Th e final cha pter present s 32 illustrati ve clinical case s with pulm onary funct ion test results and flow-volume curves, follow ed by review questions; the answers and a comm entary appear on the reverse side of the last page of eac h case study. Strengths: Th e 32 clinical cases are reason enough to read this handbook. These cases provide much the same value as a 2-week "rotation" in a busy pulmonary function laboratory. Jargon and mathematical data are minimized . Th e authors advoca te recognition of the pattern s of common abnormalities; thus. readers can ignore most of the numb ers printed on the pulmonary function reports. Deficiencies: The text contains fewer than 10 references. and the useful American Th oraci c Society standards for pulm onary function testin g are not included. In addition, the authors do not address such topics as the importance of changes in lung function detected on follow-up ex aminations and guide lines for the substitution of pulse oximetry for arterial blood gas measurement s. Reco mme nded Readership: Medical students. med ical residents. pulmon ary fellows, and primary -care physician s will find that this handb ook impro ves their efficient use of obj ective pulm onary function tests, which are underused by most clinicians. Conte nts: The book is divided into two main sections, Classic Conn ective Tissue Diseases and Miscell aneous Disorders, and has an introduction . The introduction. an intere sting discussion of the history of and rationale for the nomenclatu re and classification of rheum atic disease. provides some insight into our current perception s about these disorders. The section on the classic conne ctive tissue diseases include s chapters on rheum atic fever. rheumatoid arth ritis. systemic lupus erythematosus. scleroderma and CR EST (calc inosis cut is. Rayna ud's phenomenon. esophagea l dysfunction. scle rodactyly, and telangiectasia) syndrome, dermatopolyrnyositis, Sjogren ' s syndrome, relapsing poly chondriti s. and the vasculitides. The miscellaneous section reviews amylo idosis, Behcei' s disease, Buerger' s d isea se, Cogan's sy ndro me, eosi nophilic fasc iitis, eosinophilia-mya lgia syndrome. toxic oil syndrome. familial Medi terranean fever. Kawasaki syndrome, Lyme disease, sarcoidosis, vasc ulitis " look-alikes," and the pseudo vasculiti s syn dromes . Each chapter contains interesti ng and often obscu re historical notes on the disease, follo wed by a discussion of the clinical manifestations and histopathologic features with corresponding illustrat ions. The usefulness of speci fic tissue biopsy for different ial diagnos is is usually discussed. Stre ngt hs: Thi s innovative book is unique. Before its publication, physician s had to consult seve ral sources to determine what could be gleaned or concluded from a patient's biopsy specimen in order to diagnose a systemic inflammatory or connec tive tissue disease. The text is interestin g, practical, well written. concise. and abundantl y illustrated. Deficiencies: Although it would be expensive. the use of color illustrations would have enhanced the presentation. Cov erage of other rheumatic co nditions such as osteoarthritis wou ld have been helpful; perhaps the publi sher could present this information in a co mpanion volume in the same series. Recom m ended Readership : Th is book would be appropriate for rheumatology fellows . internal medi cine residents. internists, and other physician s who are interested in and routinely treat patient s with rheumatic diseases.
