Type ofBook: A multiauthored text on liver disease. Scope of Book: This volume presents a concise, yet comprehensive, overview of the diagnosis and management of liver disease. Contents: The book contains 35 chapters, organized into 4 sections devoted to normal hepatic function, evaluation of patients with liver disease, diseases of the liver and biliary tree, and specialized evaluation of the efficacy of therapy for various biliary diseases. The chapters are written by generally acknowledged experts in the field. The writing style is clear, concise, yet complete, which is a difficult balance to achieve. All the major liver diseases are covered. The text is remarkable not only for its clarity and overall balanced coverage but also for the rich illustrations, figures, and drawings. These features make the text more readable, underscore concepts, and illustrate points while providing visual interest. Another asset is the section on the specialized evaluation of treatment, which reviews information not routinely found in other texts, ie, chapters on surgery, evaluation of surgical risk in patients with liver disease, and endoscopic management. Strengths: The strengths of this book include the balance that it achieves between the concise, easy-to-read writing style, the rich illustrations, and the completeness of the presentation. Deficiencies: I noted no deficiencies. Recommended Readership: 1 recommend this text for students, residents, fellows, and attending physicians who are interested in hepatology. It will be particularly appealing to practicing gastroenterologists and hepatologists. Overall Grading:

Type of Book: A multiauthored textbook on diseases of skeletal muscle. Scope ofBook: The goal of the book is to provide a broader view of the disorders of skeletal muscle than is available in subspecialty texts and in greater depth than that provided by general texts.

Contents: The 3 sections of the book survey characteristics of normal muscle, diseases of muscle, and approaches to the patient. The first section describes the structure and development of muscle, provides a synopsis of muscle biology, physiology, and biochemistry, and reviews muscle function. Five chapters in the second section address the various types of inflammatory disorders of muscle, and individual chapters discuss metabolic myopathies, congenital myopathies, neuropathic disorders and muscular dystrophies, fibromyalgia and related disorders, and rhabdomyolysis. The third section reviews examination of the patient, basic clinical and laboratory tests required for a generic diagnosis, the usefulness of autoantibody tests, electromyography, the role of muscle biopsy in establishing a specific diagnosis, and the rehabilitation of patients with neuromuscular disorders. Strengths: The book summarizes a great deal of clinical and basic science information and is easy to read. The chapters on muscle function and rehabilitation provide nice summaries, and the text contains numerous useful schematic diagrams. Deficiencies: The coverage of topics is uneven-l 57 pages are devoted to inflammatory muscle diseases, 25 pages describe autoantibody tests, but only 24 pages are allotted to the numerous neuropathic disorders and muscular dystrophies. In addition, the diagnostic criteria for dermatomyositis and polymyositis are lumped together. The light micrographs are of uneven quality, and the chapter on the muscle biopsy is incomplete. Recommended Readership: This book would be appropriate for medical students, residents, general practitioners, and subspecialists interested in inflammatory muscle diseases. Overall Grading:

Type ofBook: The first edition of a comprehensive multiauthored textbook on carotid artery surgery. Scope of Book: This book is intended as a teaching text and reference source, primarily targeting surgeons involved in the treatment of carotid artery disease. Contents: The text has 55 chapters divided into 6 sections. The first section consists of 6 chapters on diagnostic imaging and pathology. Section 2, Preoperative Evaluation of Carotid Disease (11 chapters), encompasses a myriad of concerns during preoperative evaluation. The 17 chapters in section 3, Surgical Management of Carotid Disease, cover basic principles of repair, difficulties with anatomical location and exposure, surgical techniques and types of repair, and the mechanisms of blunt and penetrating carotid injury. Section 4, Perioperative Monitoring and Management During Carotid Endarterectomy (15 chapters), addresses the use of perioperative adjuncts such as intraoperative ultrasonography, cerebral blood flow studies, electroencephalog-

raphy, and shunting and the management of intraoperative or immediately postoperative complications. Postoperative care is the focus of the 3 chapters in section 5, primarily long-term management, follow-up, and medical issues specific to patients who have undergone carotid endarterectomy. Section 6 (3 chapters) is devoted to the future of carotid endarterectomy, including economical considerations and a comparison of the results of carotid endarterectomy trials vs future trials of endovascular stenting. Strengths: The senior editor has supplemented his previous single-authored text on principles and technique with comprehensive multiauthored contributions integrating medical, radiographic, and surgical spheres to produce an excellent surgical tome covering nearly all aspects of carotid artery surgery. The text deals especially well with surgical controversies, as exemplified by the editors' statement in the preface that "multiple presentations of some material are usually valuable in presenting alternate technical points of view ... to completely cover the field ." Deficiencies: This text suffers, as do most multiauthored texts, in the area of up-to-date coverage of rapidly evolving fields . For example, the chapter on computed tomographic angiography does not discuss 3-dimensional reformatting, and the chapter on magnetic resonance angiography neglects the bolus gadolinium contrast technique. The text is also weak on surgical anatomy, both in diagrams and in discussions. Recommended Readership: This text would be an excellent addition to the libraries of students and practitioners of carotid artery surgery. Other health care professionals would find selected chapters informative, and the book could easily serve as a reference source on management controversies. Overall Grading:

The first section provides an overview of the biopsychosocial model of chronic pain and pain in general. Several theoretical aspects of pain are reviewed. Part 2 includes in-depth discussions of special populations and topics such as low back pain, temporomandibular disorders, fibromyalgia, cancer pain, human immunodeficiency virus and acquired immunodeficiency syndrome, issues regarding the workplace, and the effects of stress on chronic

pain . The third section focuses on the prevention and management of chronic pain , with chapters on screening, vulnerability to pain , the workplace, and chronic pain . Strengths: This book is a comprehensive and well-delineated review of current issues in pain theory and treatment, with particular emphasis on chronic pain . The editors and contributing authors present current research and theory in a succinct, practically oriented text. Deficiencies: I noted no major deficiencies. Recommended Readership: Psychologists, residents, fellows, and staff physicians involved in the area of pain medicine.
