Leprosy is an infectious disease endemic in several countries. In Morocco, this disease is decreasing. It presents mainly with cutaneous and nervous lesions, but osteo-articular locations can be observed. We report the case of a patient having a chronic polyarthritis which revealed leprosy [1]. The patient, 50 years old, presents since 1 year with a polyarthritis of the large and small joints, ulcerations of the hands and the feet, Raynaud’s phenomenon and sexual impotence. Clinical examination found a leonine facies, 20 painful joints, six synovitis, fingers in claw, ulcerations of fingers and toes with infection and necrosis, and hypoesthesia in gloves and in socks. Erythrocyte sedimentation was 35 mm/h and the C-reactive protein level 102 mg/L. Glycemia level normal and syphilitic serology negative. Radiographs of the

hands and anteroposterior of the feet showed longitudinal osteolysis of the distal phalanxes of the right index (Fig. 1), big toes and 5th left toe (Fig. 2). A diagnosis of nervous leprous osteoarthropathy was raised and the patient had been sent to specialized centre.
