To the Editor: We thank the authors for their interest in our paper. They pose a conceptually interesting and potentially practice-changing question. Regrettably, our retrospective review did not include information on the etiology of the carcinomatous vulvar lesions. HPV testing of vulvar cancers is not routine at our institution, and the presence of lichen sclerosus was not documented consistently enough to allow confident

conclusions about the risk of recurrence or the possible benefits of treatment of lichen sclerosus with topical steroids. One of our inclusion criteria was the presence of invasive cancer, and we have no data relevant to the authors' discussion of management of VIN. We believe our core conclusion: that women with close but clear margins can be managed expectantly. We did suggest that further prospective research is needed, and the authors' suggestions and ongoing research are one of those possible avenues. We look forward to the release of the manuscript that the authors mentioned describing the clinical course of HPV-negative vulvar cancers.
