Because the histopathologic criteria of parathyroid carcinoma keep changing and are not settled, it is wiser to review the histopathology of patients before including them.
To the Editors: We read with interest the article ‘‘Is Central Lymph Node Dissection Necessary for Parathyroid Carcinoma?’’ by Hsu et al.1 We congratulate the authors for addressing an important controversial issue related to parathyroid carcinoma. The incidence of parathyroid carcinoma is very low, and we accept that the deficiencies in the Surveillance, Epidemiology, and End Results database. Having mentioned this, we have a few comments and questions. In contrast to others,2,3 Hsu et al1 concluded that lymph node involvement does not affect the overall survival in tumors less than 3 cm but whether the tumor size predicts parathyroid carcinoma in the absence of local invasion and lymph node involvement is not clear. We want to know how many patients had a histologic occurrence of parathyroid carcinoma and did they differ in terms of recurrence and disease-free survival so we can tell whether these patients will need a repeat operation.
