Health Status problems were put in priority or&r by a tri-participant co&ty agency/provider/technical agency effort. Subsequent steps involved setting goals and objectives and consideration of alternative programs of action. A significant result was successful solicitation of local government funding for a Hypertension Resource and Education Center operated by a community organization of providers and consumers. The center’s goals are to: (1) increase awareness of the dangers of untreated high blood pressure among members of the community (2) motivate through education, counseling and follow-up the individual’s assumption of self-care behavior for high blood pressure control, (3) share and coordinate existing community resources for high blood pressure control through the formation of a consortium of concerned agencies and individuals, (4) increase accessibility of the population to screening/referral/education programs, (5) increase the percentage of hypertension residents who remain under medical supervision/treatment, (6) generate the participation of various groups, such as school populations, community organization, churches and senior citizen’s groups in achieving the above goals, and (7) share information with the HSA in the development of the Health Systems Plan and the Annual Implementation Plan.

Under a State contract, a technical advisory agency and a community organization evolved a process for planning interaction between Health Systems Agencies (HSA’s) and Subarea Councils (SAC’s) resulting in a community supported choice of essential hypertension as a top priority health status problem. The Hypertension section of a major university hospital collaborated in subsequent steps of setting goals and objectives and considering alternative programs of action. Chosen goals targeted on preventive measures, stress, life style and control of HBP. Objectives related to accurate primary data collection, awareness, treatment, inadequate Recommendedactions focused on educationcontrol and cerebrovascular death rate. and coordination of al programs, surveys generating primary data, docmentation available resources, detection, confirmation and referral.
