thalmus, as distinct from conventional neurons in the rest of the central nervous system, seems firmly established. A new dimension of peptidergic activities has been introduced by the demonstration of release sites of bioactive neuropeptides in more or less close spatial relationship to various effector cells. Even neurons can be addressed by neuropeptides in a directed manner. The conveyance of chemical signals to neuronal effector cells over a distance, that is via the acellular interstitium, represents a new form of neuron-to-neuron signaling. The concept of neuromodulation is that of a signal transmitted synaptically between 2 conventional neurons being either enhanced or depressed by an opioid peptide from a third neuron. The degree of specificity and diversity attained by the peptidergic control systems in operation can be understood on the basis of the rich supply of various peptide principles available and the capacity of 1 neuropeptide to function in several different ways depending upon where it is put to use. Each patient had a duplex system and 2 had unusual complications: 1 with a ureterovaginal fistula after vaginal incision and drainage, and a 79-year-old patient with adenocarcinoma of the ureterocele and the bladder. 6 figures, 18 references

Abstracter's comment. This is a reminder of the potential for incontinence resulting from incision of the blind-ending lumen of a cecoureterocele that extends distal to the bladder as a tongue beneath the urethral submucosa. A different but comparable problem is that of a ureteral orifice with an ectopic urethral or vaginal location that continues to communicate with the bladder despite proximal surgical treatment. M. G.
