A patient with tubular supravalvular aortic stenosis presented with a progressively enlarged right coronary artery (RCA). Magnetic resonance imaging (MRI) revealed a diameter of proximal aorta and RCA of 1.3 cm and 5.5 cm, respectively (A). Surgery was indicated to reduce the pressure gradient of more than 100 mm Hg across the aortic stenosis and to prevent further coronary artery enlargement with imminent rupture. Intraoperatively, the RCA presented as a giant aneurysm with diameter and length exceeding 5 cm and 8 cm, respectively, then winding along the right ventricle (B). We replaced the proximal aorta by a modified Doty extended aortoplasty. The RCA was reconstructed by exclusion of excessive cavity (D). A postoperative MRI shows the reconstructed RCA and the Dacron aortic graft with well-preserved sinu-tubular junction (C). Echocardiography ruled out aortic valve incompetence or any remaining gradient along the aorta, and the patient was soon discharged.
