Dental units and water retraction Council on Dental Materials, Instruments, and Equipment

he concern that water retraction in dental units may compromise infection control practices has been expressed by the Council on Dental Materials, Instruments, and Equipment; the Council on Dental Therapeutics1; and the Centers for Disease Control.2'3 In an effort to assist the dental profes­ sion, the Council on Dental Materials, Instruments, and Equipm ent conducted a survey on dental units and water retrac­ tion. Inform ation on some older units

(Table 1) and currently available units (Table 2) is included. The tables summa­ rize the responses from the companies. The tables indicate the m anufacturing or supply company, the model of dental unit, and the presence or absence of water retraction, the extent of water retraction (if any), whether it meets the requirement 3.5 in ANSI/ADA specification no. 47 for dental units,4 possible conversion, check valve, valve type, and whether the war­ ranty status is affected by the conversion.

Requests for information on dental unit water retraction or conversion should be directed to the specific company. As the degree of risk of cross-contamina­ tion from units with water retraction is not known, it is prudent for handpieces and other items connected to the waterlines of dental units to run and discharge water into a sink or container for at least 2 m inutes after com pleting care on each patient. Manufacturers’ recommendations should be followed for use and m ain­ tenance of waterlines, check valves, and for the flushing of handpieces. T he current ANSI/ADA specification no. 47 for dental units requires that dental units shall incorporate means of preventing water from being drawn back beyond the c o n n e c tin g p o in t of the handpiece to the handpiece tu b in g at shutoff. T his requirement, together with handpieces that can be sterilized, provide significant infection control practice in preventing cross-contamination. A method to test for the presence of water drawn back beyond the connection point of the handpiece to the handpiece tubing is described in the specification. 420 ■ JA DA, Vol. 116, M arch 1988

“Connect a 6 inch (15.2 cm) length of 1/16 inch (0.159 cm) inside diameter trans­ parent tube to the fitting to which the water tube of the handpiece is normally attached. T he unconnected end of the 6 inch (15.2 cm) tube must be squared off. Operate the u n it’s water valve as it would normally be done during handpiece oper­ ation and in accordance with the manufac­ tu rer’s specifications. Operate until a continuous stream of water flows from the 6 inch (15.2 cm) long tube; then shut water valve off as it would normally be done when shutting the handpiece. The meniscus of the column of water in the tube shall not exceed 0.800 inches (2.032 cm) from the end of the tube when held vertically, with the open tube end ex­ tending upward.”4 T he dental industry is invited to sub­ m it dental units for evaluation by the
