simultaneous ring-shaped contractions were evident (B); a spiral, screw-like contraction was observed from the distal esophagus to the esophagogastric junction (C). Manometry revealed simultaneous, nonperistaltic contractions in the body of the esophagus (D). The lower esophageal sphincter pressure was slightly elevated (60 mm of water). A diagnosis of diffuse esophageal spasm was made, and the patient was treated by balloon dilation twice per week. After 5 treatments, the symptoms were resolved.
