Despite an extensive international clinical experience, the management of thyroid and parathyroid disorders continues to generate considerable controversy. Issues include indications for unilateral versus bilateral parathyroid exploration in the management of hyperparathyroidism, the role of pre-operative imaging studies in diseases of the thyroid and parathyroid, total versus less than total thyroidectomy for papillary carcinoma, and the indications for postoperative I-131 therapy in the management of welldifferentiated thyroid carcinomas. New techniques in radiologic imaging, pathologic analysis, molecular biology, and surgical approaches have altered many of the traditional paradigms. Contemporary management of this group of disorders now requires a multidisciplinary team consisting of surgeons, endocrinologists, nuclear medicine specialists, and pathologists. In this issue of the Otolaryngology Clinics of North America we provide an update on the clinical management of thyroid and parathyroid disorders. Concepts in anatomy, laboratory analysis, radiologic imaging, surgical approaches, and medical management are reviewed.
