The authors conclude that OCD and AVN are pathologic entities with definite similarities, possibly representing a spectrum of the same pathophysioiogy, necrosis of bone. The authors believe that many cases of acquired malocclusion, facial deformity, and condyiar degeneration may be the consequence of AVN. Further, they assert that OCD and AVN are common and often associated with preexisting internal derangement of the temporomandibuiar joint. The authors advise MR imaging of the TMJ with suspected AVN or internal derangement.-S.J. mors originating in the buccal mucosa. The treatment modalities described included radiotherapy, surgery and radiotherapy, and surgery alone. Surgical intervention varied from wide local excision to therapeutic radical neck dissections depending on the previously stated prognosticators, the clinical situation and the individual patient. It is recommended that local excision be the initial therapy in the treatment of minor salivary gland tumors. A margin of normal tissue is adequate for benign tumors, but due to an increased rate of recurrence, wide resection is advised with malignant lesions. Finally, based on prior experience showing decreased evidence of recurrence in malignancies of the major salivary glands, the authors also recommend the use of combined surgery-radiotherapy in the treatment of malignant minor salivary gland tumors.-F. Distinctive features of intercalated duct carcinomas are their biological low grade, whereas excretory duct carcinomas are high-grade malignancies. Salivary gland adenocarcinomas occur less frequently than adenoid cystic or mucoepidermoid carcinomas. The intercalated terminal duct adenocarcinomas are found preponderantly in the oral cavity, especially the palate. Excretory and interlobuiar salivary duct carcinomas are almost exclusive to major salivary glands, especially the parotid.-G.H.

Minor Salivary Gland Tumors of the Lip and Buccai Mucosa. Weber RS, et al. Laryngoscope 99:6, 1989 The varied presentation, prognosis, and treatment of minor salivary gland tumors of the lip and buccal mucosa is described. Fifty patients with minor salivary gland tumors of the lip and buccai mucosa were treated and followed over a 41-year period at the M.D. Anderson Cancer Center. Ail patients were divided into three groups dependent upon the type of treatment they had received before referral. Group 1 patients were previously untreated, Group 2 patients had undergone previous incisionai or excisional biopsies, and group 3 had received surgery and local radiotherapy. The median age for presentation for lip and buccai mucosa tumors was 65 years and 48 years, respectively. The most commonly described presentation was a palpable submucosai mass. In this series, malignancy predominated at both sites (84% of lip lesions and 100% of the mucosal tumors) with adenoid cystic carcinoma being the most common histologic finding followed by mucoepidermoid carcinoma. The prognosis of the lesion is shown to be affected by the histologic form of the tumor, the presence of perineurai involvement, and the site of the primary tumor. Adenoid cystic carcinoma, the most frequently seen tumor (42% of the tumors; 21/50 patients), accounted for all the recurrent lesions. The presence of perineurai involvement was also associated with an increased rate of recurrence. The authors present a technique for the reduction of a prominent mandibular angle in patients whose square and muscular jaw is unappealing. Forty-two consecutive female patients were treated. Twenty-four had resection of bone alone, and 18 had resection of bone and muscle. Preoperative workup included frontal, profile, and submental photographs, and cephalometric, panoramic, and AP radiographs. The amount of bone to be resected was determined by examining the photos to decide the amount of contouring desired, then extrapolating this by comparison with the AP and panoramic films. The resection was accomplished through an intraoral incision over the anterior portion of the ascending ramus. After retracting the masseter muscle, the lateral mandibular angle was marked and resected. Postoperatively a pressure dressing was applied for two days.-P. Effects of Intralesional Versus Intraperitoneai Administration of Cispiatin on Squamous Cell Carcinoma of Mice. Nomura T. Jpn J Oral Maxiiiofac Surg 35:375, 1989 The effects of intralesional (IL) versus intraperitoneal (IP) administration of cispiatin (12 mg/kg to 24 mg/kg, single injection) were studied using transplantable squamous cell carcinoma in mice. The size of the tumor in the control and saline-injected groups increased approximately 20 times and 70 times at 3 and 6 weeks, respectively, whereas tumor growth was inhibited by more than 50% in the cispiatin-treated groups at 3 weeks. The inhibition was more pronounced in the IL groups than in the IP groups at any stage of experiment. As compared with the IP groups, relative tumor volumes in the IL groups receiving 12 mg/kg and 18 mg/kg of cisplatin were <50% at 3 weeks. Histologically, focal necrosis of the tumor tissue was seen only in the IL groups from one day to 1 week at the dose of 12 m&g. Necrosis of individual tumor nests was also observed more frequently in this group than in the IP group, but in areas of residual tumor,

swelling of the nucleus, disappearance of the nucleolus, nuclear vacuolization, decrease of nuclear stainability, and occurrence of giant, bizarre-shaped nuclei were seen almost equally in both groups. At the dose of 18 mg/kg there was no significant difference in these changes between the two groups. Although necrosis of individual tumor nests was seen in all animals with IP administration, focal necrosis of the tumor tissue was not encountered in any animal, whereas focal necrosis was seen in three animals in the IL group. Almost complete tumor necrosis was seen in four animals in the IL group and one animal in the IP group. The mitotic index fell below 0.5 in all cisplatin-treated groups, but no statistical difference was noted between IL and IP administration. Flow cytometric analysis of the nuclear DNA content showed a

tendency to decrease in the S and G2M phase cell populations in the IL group and increases in the IP group. There were no differences in weight loss and histologic changes in the major organs between the two groups, but in contrast to 30% for the IP group, the rate of toxic death for the IL group was 5% at 18 mg/kg. The results indicate that IL administration may produce a better response of the tumor to cisplatin, with less adverse effects, than systemic administration, possibly by maintaining higher levels of the agent in the tumor tissue.-T.
