misses the new wave carrying the anatomy and neuropharmacology of the enteric nervous system to the forefront of these prevalent problems. Clearly, a clever entrepreneur may yet provide "on-line" updates as knowledge supplements to a soundly conceived basic text. The authors' commitment to a text of clinical value is evident in the method of ordering and presenting tabulated and photographed information. Schematic drawings are detailed and clear, line drawings that sequence clinical thinking are frequent, (I almost lapsed into mentioning "critical pathways"), and tables are relevant and readable. The bibliography of the first edition is largely maintained, but each chapter is supplemented by a reference list drawing from 1992-1993 literature. It is always wise to know which audience is best served by a publication. The editors were very deliberate in sculpting a body of knowledge of clearest use to general physicians and surgeons, residents, and fellows in training, and, I assume, to advanced nursing personnel often active in the ambulatory setting. Good and serious editing maintained this purpose and

created a coherent style that makes for seamless reading. In my view, they have achieved even more; a reminder that good doctors think clearly and that good teachers are an enduring and economical form of health insurance. JOEL B.
