June 198~ FAILURE ,~Z)DES AND EFFECTS ANALYSIS (FMEAIFMECA)

The R e l i a b i l i t y Analysis Center offers an FMEA/FMECA service to OoD, DoD contractors and private industry. The RAC mission is to serve as a technological resource; a center of excellence in the description of electronic r e l i a b i l i t y and product assurance and we have an experienced staff ready to perform comprehensive Failure Modes and Effects Analysis. FMEA/FMECA - Failure Modes and Effects Analysis/Criticality Analysis (FMEA/FMECA) are r e l i a b i l i t y analyses that identify potential hardware and software design deficiencies. The FMEA is a procedure which documents a l l probable f a i l u r e s in ~ system w i t h i n specified ground r u l e s , determines by f a i l u r e mode analysis the e f f e c t s of each f a i l u r e on system operation, and c l a s s i f i e s each by s e v e r i t y . The FMECA is an FMEA combined with a c r i t i c a l i t y analysis. The c r i t i c a l i t y analysis is an analysis procedure for associating f a i l u r e p r o b a b i l i t i e s with each f a i l u r e mode. The FMEA/FMECA can help an organization in the following ways: e e e e e • e e

Provide the organization with another viewpoint on a design Aid in p r o j e c t i n g warranty and q u a l i t y costs Aid the designers in producing a product with a lower MTBF Aid Customer Service in w r i t i n g a f i e l d r e p a i r manual Aid Software Engineering in more complete d i a g n o s t i c software Provide the designers with an undetectable f a i l u r e l i s t Provide the designers with a ranked c r i t i c a l parts l i s t Provide the designers with a s i n g l e point f a i l u r e s l i s t

RAC performs functional and hardware style FMEA/FMECAs on electronic and mechanical equipments. Included with these analyses are MIL-HDBK-217D Parts Count Analysis (PCA) and Parts Stress Analysis (PSA) f a i l u r e rate predictions. The PCA is automated with Powertronics RPP-D software. The FMEA/FMECA is autemated and performed on the dBase I I database management system and is deliverable on 5~" and 8" floppy disks with CP/M-80 and MP/M formats (other formats w i l l be available soon). Worksheets are also available in hard copy form. The FI~EA/FMECA techniques are based upon those outlined in MIL-STD1629A and MIL-HDBK-338. The s p e c i f i c techniques are t a i l o r e d to i n d i v i d u a l equipments and t h e i r missions. FMEAIFMECAt r a i n i n g both ons i t e and at the RAC is a v a i l a b l e upon request.
