A 36 year old male patient had a rope slide accident. He sustained a left bimalleolar fracture dislocation and underwent osteosynthesis. Two years later, arthroscopy of the ankle joint was performed to remove symptomatic osteophytes. Soon after, a pulsatile mass developed on the dorsum of the foot. Magnetic resonance imaging (red arrow, panel A) and colour duplex sonography (panel B) demonstrated a 32 cm false aneurysm of the dorsal pedal artery. Direct suture of the neck of the aneurysm was possible with preservation of normal arterial flow. The post-operative ankle brachial index and distal oscillograms were normal and symmetrical.
