We employed the following techniques: - Semi-standardized psychological interview - Komel index for neurosis - Mahover test. With these techniques we obtained results which indicated: - degree of neurosis - frequency sequence of individual neurotic trends - existence and differentiation of defence mechanisms Low degree neurosis was observed in 35%, medium degree in 50% and high degree in 15% of subjects As to the frequency of neurotic trends, anxiety trends were most common followed by phobias, depression and obsessive compulsory trends. The most frequent defence mechanisms were regressions (high percentage), followed by aggression and reactive formations.

Using RT/PCR, we showed that freshly plucked human anagen hair expressed both type 1 (80 Kd) and type 2 (60 Kd) IL-l-Receptors mRNAs. The IL-l-Receptor type 1 was functional since after in vitro stimulation of plucked hair with IL-la, we observed the induction of mRNAs for the inflammatory cytokines IL-lp, TNFa and IL-6 as well as for the chemokines MCAF, MCP-3 and IL-8. In addition, the growth of dissected human anagen hairs in culture in vitro was significantly and dose-dependently (0.01-10 rig/ml) inhibited by IL-la. In order to identify individuals with high inflammatory potential in their hair follicles, we designed a rapid and simple assay to detect variations in the level of IL-la production in the overnight supematant of plucked hairs in vitro. We observed that 33% of the specimens the volunteers tested (n = 116) could be considered highly inflammatory in terms of IL-lo production. Altogether, these results suggest that anti-IL-l strategies might be of therapeutic relevance not only in Alopecia A reata but also in Alopecia androgenetica, at least for the highly inflammatory individuals. length of duration of disease and type of alopecia. The lowest activity of monoxygenase system has been found in patients with alopecia universalis as compared with alopecia areata. A new method of treatment (photochemotherapy+ inductors of monoxygenase system) has been utilised.

Increased sensitivity of the pilosebaceous unit to androgens, with regular ovulatory menstrual cycles and normal serum testosterone and debydroepiandrosteronesulphate levels is usually defined as idiopathic hirsutism. Increased 5-alpha reductase enzyme activity converting testosterone to dihydrotestosterone in the hair follicle appears to be the major mechanism underlying the abnormal skin sensitivity to androgens. Finasteride, a specific inhibitor of 5-alpha reductase, recently has been investigated in the treatment of idiopathic hirsutism. In an attempt to compare the efficacy of treatment, 40 women with idiopathic hirsutism were prospectively randomized to receive either 5 mg finasteride or 100 mg spironolactone for 6 months in a single blinded study undertaken in a tertiary hirsutism clinic. Hirsutism scores were measured according to FerrimanGallwey (FG) scoring system and side effects were monitored for 6 months of treatment. Blood samples were taken at each visit for assessment of endocrine, biochemical and hematologic parameters. FG scores were decreased significantly in both groups at the end of 6 months. The percent of change in scores in finasteride and spironolactone group were as follows: 5.9 1% & 7.18% and 20.60% & 12.59 at 3 months, 10.61 i 12.18% and 32.57% f 15.68% at 6 months, respectively. There was a significantly better response with spironolactone treatment at the end of 6 months (p < 0.0001). No side effects were observed during finasteride treatment. Irregular bleeding was the most common side effect experienced by ten patients (50%) in the spironolactone group. However, none of the subjects withdrew from the study because of side effects. The present data suggest that both finasteride and spironolactone were effective in the treatment of idiopathic hirsutism. However, it appears that spironolactone group responded significantly better.

This study was aimed to determine the role of monoxygenase system in the pathogenesis of alopecia. 43 patients with alopecia were included in this study. Patients were classified according to their age of onset, duration, type of alopecia. Antipyrine halflife in alopecic patients was found to exceed by 32%. The levels in healthy controls evidence of inhibited monoxygenase system in alopecia. Activity of monoxygenase system correlated with

During the catagen the mitotic activity of the matrix ceases, but metabolic and hormonal activity of the sac-cells increases so as to prepare the follicle for a new anagen. During the catagen the sac-cells must produce oestrone, cortisone and glycogen. Oestrone is formed by aromatisation of the precursors: testosterone, androstenedione, oestradiol; cortisone is actively metabolised by cortisol; glucose produces glycogen which accumulates in the exterior shoath.
