A 36-year-old woman presented complaining of increasing hoarseness of voice for 3 months. She denied any history of chest trauma, thoracic surgery, or central venous access. Contrast-enhanced computed tomography angiogram revealed an 80 55-mm saccular aneurysm arising from the anterolateral surface of the superior vena cava (SVC; A [arrow]). The SVC aneurysm was exposed via a midline sternotomy; surrounding mediastinal fat and other structures were noted to be normal (B). The aneurysm was successfully excised and the subsequent SVC defect primarily sutured. The patient had a smooth recovery and her voice improved after the surgery.
