Chromoendoscopy with targeted biopsies is the optimal surveillance colonoscopy modality in patients with inflammatory bowel disease (IBD). Targeted as compared to random biopsies and chromoendoscopy as compared to white light endoscopy show improved yield for dysplasia detection (Laine et al1). One of the main barriers to the implementation of chromoendoscopy in routine clinical practice has been the difficulty to obtain and use the dye (Sanduleanu et al2). We provide a summary figure (Figure 1) and table (Table 1) as a guide to facilitate this step.

Figure 1. A, To prepare the dye, the indigo carmine (IC) or methylene blue (MB) solution is diluted with sterile water. B, Alternatively, the IC powder is mixed into sterile water. C, The IC or MB is reconstituted by gently swirling the sterile water bottle. D, the water bottle is connected to the water jet channel using the auxiliary foot pump. E, an example of the color achieved with dilution and reconstitution of the dye. F, The dye solution is sprayed onto the antigravity side of the colonic mucosa using the water jet channel in a circumferential motion while withdrawing the colonoscope.
