ANA'IXIMY(ICD-0 180) Primary Site. The cervix is the lower third of the uterus. It is roughly cylindrical in shape, projects through the upper, anterior vaginal wall, and communicates with the vagina through an orifice called the external OS. Cancer of the cervix may originate on the vaginal surface or in the canal. Nodal Stations. The cervix is drained by preureteral, postureteral and uterosacral routes first station nodes: Parametrial, into the following hypogastic (obturator), external iliac, presacral and common iliac. For staging purpose the following examination methods are permitted: Palpation, inspection, colposcopy, endocervical curettage, hysteroscopy, cystoscopy, proctoscopy, intravenous urography, and X-ray examination of the lungs and skeleton. Over the last several decades there has been continued confusion about the stages of preclinical invasive carcinoma of the cervix. Several classification systems have been developed which have not been generally satisfactory. The addition of colposcopy in many countries has caused further confusion as to what is a clinical lesion for obvious reasons. There has also been an increased pressure to put measurements into the definition. As a result of the above a new definition was proposed in volume 20 which is as follows: Stage Ia Carcinoma should include minimal microscopically evident stromal invasion as well as small cancerous tumours of measurable size. Stage Ia should be divided into those lesions with minute foci of invasion visible only microscopically as Stage Ial, and the macroscopically measurable micro-carcinomas as Stage Ia in order to gain furhter knowledge of the clinical behaviour of these lesions. The term Ib Occult should be omitted. The diagnosis of both of Stage Ial and Ia should be based on microscopic examination of removed tissue, preferably a cone, which must include the entire lesion. As noted above, the lower limit of Stage Ia should be that it can be measured macroscopically (even if dots need to be placed on the slide prior to measurement) and the upper limit of Ia is given by measurement of the two largest dimensions in any given section. The depth of invasion should not be more than 5 mm taken from the base of the epithelium, either surface or glandular, from which it originates. The second dimention, the horizontal spread, must not exceed 7 mm. Vascular space involvement, either venous or lymphatic, should not alter the staging, but should be specifically recorded as it may affect treatment decisions in the future. As a rule, it is impossible to estimate clinically whether a cancer of the cervix has extended to the corpus or not. Extension to the corpus should therefore be disregarded.
30 CERVM A patient with a growth fixed to the pelvic wall by a short and indurated but not nodular parametrium should be allotted to Stage IIb. It is impossible, at clinical examination, to decide whether a smoth and indurated parametrium is truly cancerous or only inflammatory. Therefore the case should be placed in Stage III only if the parametrium is nodular out on the pelvic wall or if the growth itself extends out on the pelvic wall. The presence of hydronephrosis of the ureter by cancer permits according to the other findings, Stage II.

The presence of bullous oedema, as such, should not permit a case to be allotted to Stage IV. Ridges and furrows into the bladder wall should be interpreted as signs of submucous involvement of the bladder if they remain fixed to the growth at palposcopy (i.e. examination from the vagina or the rectum during cystoscopy). A finding of malignant cells in cytologic washings from the urinary bladder requires further examination and biopsy from the wall of the bladder.
Cases should be classified as carcinoma of the cervix if the primary growth is in the cervix. All histologic types must be included. Grading by any of several methods is encouraged but is not a basis for modifying the stage groupings. It is desirable that all tumours be microscopically verified, but cases that clinically are likely to be cancer without such confirmation should be included with special attention to descriptive detail. The number should be kept to a minimum. RUMS

The increasing complexity of treatment regimens has resulted in many Hospitals care at more than one institution. patients receiving contributing data to the Annual Report should include all cases seen whether or not all therapy was rendered at with a view to treatment, the reporting hospital. For example: Patients referred for treatment outside the reporting institution (i) should be included in the statement provided that treatment was originally planned at the reporting institution, and that the patients have been referred back for follow-up or did not survive the treatment. Patients examined and/or primarily treated at another institution may be included in the statement on condition that both examination and treatment have been carried out according to an agreed protocol, and that all patients examined and treated according to this protocol are included in the statement.

Altogether 515 125 cases of carcinoma of the uterine cervix treated in the years 1913 to 1986 incl. have been reported to the Editorial Office of the Annual Report. Fig. A.l, which is based on data from 44 clinics reporting treatment results from 1950 to 1986, incl., reviews the stage changes in stage distribution. The proportion taken by Stage I continously increases from 23.0% in the period 1950-1954 to 38.8% among cases treated in 1982-1986. During the same period the Stage II cases decrease from 38.0% to 29.8% and Stage III cases from 32.7% to 26.0%. The proportion taken by Stage IV cases is more or less unchanged.

From Table A.VI can be seen that the five-year actuarial survival rate for Stage Ib cases without regional lymph node involvement was 89.59,, but for cases with histologically proven involvement of lymph nodes the rate dropped to 59.0%. The corresponding figures for Stage IIa was 84.6% for cases lymph node metastases, without regional but only 48.9% for cases with histologically proven lymph node involvement. presentation

the
material.
Information concerning cases of carcinoma of the uterine cervix examined with a view to treatment in the years 1982-1986 has been furnished by 137 institutions. 125 collaborators have reported on individual cases and 12 have submitted reports in the form of summarized data. The total material is presented in Tables A.VIIIa, A.VIIIb and A.IX. Table A.VIIIa reviews the series reported by the different contributors. The three and five-year actuarial survival is given for institutions with individual reports, whereas the five-year crude survival is given for institutions reporting summarized statistics. The institutions, which have submitted summarized data are marked with an asterisk. Table A.VIIIb gives an account of the number of cases treated and surviving three and five years by stage for the different contributors. Table A.IX reviews the number of cases reported by the different institutions and the 3 and S-year actuarial survival by mode of treatment.
