A 64-year-old man underwent staging EUS for pre-liver transplant evaluation of

cholangiocarcinoma. EUS showed a hilar mass consistent with cholangiocarcinoma along with two small lymph nodes in the aortocaval space. EUS-guided transduodenal fine-needle aspiration (FNA) of the larger lymph node was performed. Rapid on-site cytologic examination showed

adequate specimen with no evidence of malignancy; however, numerous pear-shaped, binucleated, flagellated organisms consistent with trophozoites of Giardia Lamblia were

In this case, Giardia was incidentally diagnosed on a transduodenal FNA of an aortocaval lymph node in an asymptomatic patient representing duodenal contamination of the FNA and highlights
