Intersocietal Accreditation Commission (IAC) accreditation is granted or delayed depending on the fulfilment of several quality metrics.

Methods: Data from echo (n=3,079) and nuclear (n=1,835) lab accreditation applications submitted between 2012 and 2014, was evaluated to determine the mean age of lab equipment from each application. Lack of lab quality was determined by the number of missing quality metrics. The relationship between equipment vintage and quality was explored as an interaction term between equipment age and the number of missing quality metrics, and was incorporated into a multivariable model to predict accreditation status. Results: During the study period 49% of echo and 42% of nuclear labs were granted accreditation. For both echo and nuclear labs, there was a statistically significant trend towards an increasing lack of quality metrics with increasing quartiles of equipment age (figure). On a multivariable regression analysis, the interaction between equipment age and the number of missing quality metrics was a significant independent predictor of lack of accreditation for both echo (OR, 95% CI: 0.70, 0.68-0.73) and nuclear labs (OR, 95% CI: 0.68, 0.66-0.71). Conclusions: Lab quality is an important determinant of IAC accreditation, and equipment vintage is an effect modifier of this relationship. Contemporary equipment likely reflects a commitment to quality, for both echo and nuclear labs.
