INSTRUCTIONS FOR CATEGORY 1 CME CREDIT Certification. The American Academy of Dermatology is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians. The Academy certifies that when the CME material is used as directed, it meets the criteria for 1 hour of credit per article in Category 1 of the American Academy of Dermatology CME Award. The American Academy of Dermatology designates this educational activity for a maximum of 1 AMA PRA Category 1 CreditTM. Physicians should only claim credit commensurate with the extent of their participation in the activity. The AMA has determined that noneUS-licensed physicians who participate in this CME activity are eligible for AMA PRA Category 1 Credit. A formal policy statement regarding Category 1 Credit for enduring materials has been adopted by the Council on Education. The policy states that AAD Category 1 CME Credit for the Journal’s CME quiz is to be awarded for a 1-year period with a set expiration date at the conclusion of 1 year after the program issuance. This limitation has been instituted because CME activities are oriented to current and timely topics of possible limited longevity. Accordingly, the

Academy cannot accept answer sheets for credit beyond the 1-year limit. A minimum score of 70% must be achieved to receive AAD Category 1 Credit. This CME activity was planned and produced in accordance with the ACCME Essentials. Estimated time for reading the text and taking the CME quiz is approximately 1 hour. Instructions. Category 1 Credit can be claimed by reading the text material, taking the CME examination, and recording the answers on the perforated answer sheet, which can be found after the examination. Please record your AAD member identification number and the examination identification number in the spaces and scanning targets provided on the answer sheet. Your identification number can be found on your Academy membership card. The examination identification number can be found at the beginning of the CME examination. Your AAD identification number and examination identification number will be used to record your credit hours earned on the CME transcript system. Tear out the perforated answer sheet and print your name and address in the spaces provided. Fax to 630-598-1434 or mail to Planstone Group, 4745 Main St, #207, Lisle, IL 60532.
