32-year-old woman with no personal history of interest. Biologist by profession. The patient goes to the emergency room due to a 24 h mobile intraocular foreign body sensation. A year earlier, the subject travelled to Cameroon. A bulbar hyperemia was observed in the right eye upon examination, and lab tests showed 14 × 109 /l leukocytes (45% eosinophils), with the other parameters, including kidney and liver, normal. A parasite was extracted from the subconjunctival space of the right eye (Fig. 1), with diagnostic confirmation through microbiological analysis of adult Loa loa filariasis. The patient is treated with diethylcarbamazine, after reducing parasitemia with albendazole, with complete normalization of lab results. Loiasis is transmitted by the Chrysops fly bite, as it lays its eggs in muddy swamps. It is endemic in Central and West Africa. Most patients are asymptomatic, with evanescent subcutaneous lesions and subconjunctival migration being the most common manifestations. The most prevalent lab finding is eosinophilia, present in up to 84% of cases.
