Inversion inv(4)(p15q26) in a Squamous Cell Carcinoma of the Hypopharynx

There are few cytogenetic studies of solid tumors [1, 2]. Reports of cytogenetic analyses of malignant tumors from the upper aerodigestive tract are restricted to only three squamous cell carcinomas, two of the larynx and one of the tongue [35]. We present here the finding of a single cytogenetic abnormalityinv(4)(p15q26)--in a squamous cell carcinoma of the hypopharynx. The patient, a 71-year-old male smoker, was admitted to the Department of OtoRhino-Laryngology because of hoarseness and dysphagia of 2-3 months' duration.

He had also d e v e l o p e d d y s p n e a w h e n in a r e c u m b e n t position. At h y p o p h a r y n g o l a r y n g o s c o p y , a p o s t c r i c o i d a l h y p o p h a r y n g e a l t u m o r was found. The patient also had a regional l y m p h n o d e metastasis. A b i o p s y was taken from the p r i m a r y t u m o r and d i v i d e d into two parts, one for c y t o g e n e t i c and the o t h e r for h i s t o p a t h o l o g i c e x a m i n a t i o n . The latter r e v e a l e d a m o d e r a t e l y d i f f e r e n t i a t e d s q u a m o u s cell carcinoma. T h e t u m o r was classified as T 4 N I M O . C h r o m o s o m e p r e p a r a t i o n s w e r e m a d e from s h o r t - t e r m c u l t u r e s i n i t i a t e d after c o l l a g e n a s e disaggregation of the t u m o r b i o p s y (see [3] for d e t a i l e d description). A total of 22 m e t a p h a s e s was p h o t o g r a p h e d and k a r y o t y p e d . A n o r m a l m a l e k a r y o t y p e was f o u n d in two cells. N i n e m e t a p h a s e s had n o n c l o n a l aberrations. In the r e m a i n ing 11 cells, the k a r y o t y p e 46,XY,inv(4)(p15q26) was f o u n d (Fig. 1). A s i m i l a r inv{4) has not b e e n d e s c r i b e d in any o t h e r m a l i g n a n t d i s o r d e r 12]. Until n o w , c y t o g e n e t i c a b n o r m a l i t i e s h a v e not been r e p o r t e d in c a r c i n o m a s of the h y p o p h a r y n x . Nor h a v e s q u a m o u s cell c a r c i n o m a k a r y o t y p e s of a d j a c e n t segm e n t s of the e s o p h a g u s so far been described. Therefore, no data are available for c o m p a r i s o n w i t h the aberration f o u n d in the p r e s e n t case.
