JEJUNAL DIVERTICULUM CAUSING OCCULT BLEEDING AND INTESTINAL OBSTRUCTION

A 58-year-old man was admitted to our hospital with melena and a history of occult GI bleeding for 1 month. Upper endoscopy and colonoscopy were normal. A radioisotope Meckel’s scan and superior mesenteric arteriography were normal. He subsequently developed an upper intestinal obstruction with rectal bleeding. Push enteroscopy revealed a large clot occluding the lumen, 130 cm distal to the ligament of Treitz (A). Exploratory laparotomy revealed a single 8 cm diverticulum on the mesenteric jejunal border (B). The presence of the muscular layer (arrow) in the diverticular wall confirmed that the lesion was a true diverticulum (C). As of 24 months after surgery the patient had not had any further bleeding.
