Fig. 1 The patient is a 38-year-old woman who reported pain and progressive abdominal distension. Her medical history included a hydatid cyst in liver segment VIIthat had been diagnosed by ultrasound after a car accident 2 years before. CT scan (Fig. 1) showed evidence of innumerable lesions in the context of disseminated hydatid disease. During surgery, we observed peritoneal echinococcosis (crystalline fluid with numerous floating vesicles), massive disseminated hydatid disease and hydatid carcinomatosis (millimetric cysts disseminated on the peritoneal surface). The fluid was suctioned and we proceeded with splenectomy, pericystectomy, bilateral diaphragm resection, resection of the visible vesicles and peritonectomy with electrofulguration of the non-resected parietal peritoneum. §
