A 50-year-old otherwise healthy woman presented with profuse intermittent sweating in a sharply demarcated area on the right ulnar forearm and hand (Video; available online at www.jaad.org). Since 2 years of age, the patient had been suffering from unpredictable attacks of hyperhidrosis at this location, persisting for about an hour, and occurring at irregular intervals of a few hours to several months. A thorough neurologic investigation was unremarkable. The low and high power views of the skin biopsy specimen can be seen in Figs 1 and 2. 1.
