Educational objectives: To structure a capitated network and negotiate a capitated managed care contract and to implement the specialty network delivery system with a data system to distribute capitated funds equitably.

This course will address the use of powered instrumentation from its inception in August, 1992 to the present. Practical aspects of set-up and utilization will be combined with tips on the effective application of the device. The impact on other required instrumentation, surgical concepts, operative and postoperative care will be discussed. The technique of minimally invasive or "small hole" sinus surgery will be detailed with the resulting outcome/revision data.

Diagnosis and Treatment of Accounts Receivable Ailments DEBRADORF, MA Chicago, III.

Educational objectives: To monitor the effectiveness of the collection process and receivables management systems in their practices and to identify specific steps to improve the accounts receivable process.

Otolaryngologists in all practice settings from solo to group to academic will find this session helpful in determining the cause of increasing receivables and growing adjustments. Solutions are presented for each problem. l. Physician behavior: Surgeons' attitudes and errors can create collection headaches. 2. Accounts receivable reports: How to generate useful reports and monitor results. Samples will be provided. 3. Preoperative counseling: Eliminating problems from happening. 4. Receivables and managed care: A sign of the times. 5. Collecting by phone and mail: Tips you can use.

Managed care companies are increasingly penetrating the medical marketplace and acquiring large patient populations. They bring with them demands on otolaryngologists to sign capitated or discounted fee-for-service contracts or risk losing a large portion of their patient population. With little or no expertise, background, or database to assist them, these otolaryngologists commit to and sign contracts that include capitation or other stipulations that place them at risk. This course will summarize our 5-year experience administering a network of 60 otolaryngologists who provide services to a population of more than 400,000 subscribers. A plan of action will be outlined to assist in structuring a network and negotiating a contract to care for a capitated or fee-for-service managed care population. This will include discussion of computer requirements to provide the associated utilization analysis, measurement of quality, and appropriateness of care, and we will deal in detail with the equitable distribution of capitated funds.

Educational objectives: To discuss the indications and techniques for supe .rficial and medium-depth peels and to plan a long-term regimen f o r rejuvenating and enhancing photoaged skin in selected patients.

Interest in rejuvenating photoaged skin and achieving a more youthful appearance continues to grow among the general public. An ever-increasing variety of home-treatment programs and over- the-counter preparations is now becoming available. Concurrently, a tremendous increase in knowledge of chemical exfoliation and the development of significantly improved techniques has occurred. Our understanding of the effects of different agents, how to tailor our technique of chemical exfoliation to the particular patient, and the planning of a long-term program of rejuvenation has advanced significantly.
