GENERAL MANUSCRIPT INFORMATION 䊐 Submit four complete copies of your manuscript, including four sets of original illustrations. Please provide one computer disk with all files. 䊐 Provide concise title on a single page with authors, academic degrees, institution, city, and state, and full address, phone and fax number, and E-mail address of author to whom correspondence should be sent. 䊐 Assemble manuscript in this order: 1) Title page, 2) Abstract page, 3) Text, 4) Acknowledgments, 5) References, 6) Tables, 7) Figures, 8) Figure legends. 䊐 Number all pages consecutively, beginning with title page as “1” and ending with legends page. 䊐 Type manuscript double-spaced, single-sided on 8.5⬙ ⫻ 11⬙ paper. Leave at least 1⬙ margins on all sides. 䊐 Each reference cited in text must be listed in the References section, and vice versa. Double check spelling and publication details. 䊐 Use generic drug names wherever possible. Identify name and address of manufacturer in parentheses when a brand name is used. DISK MANUSCRIPT INSTRUCTIONS 䊐 Submit original manuscript on disk. 䊐 Submit disk with final accepted version of manuscript along with typed revised manuscript. 䊐 Put entire manuscripts (text, references, and figure legends) in one file. 䊐 Disks may be produced on IBM or IBM compatible (preferred), Apple, or Macintosh computers. Most major word processing programs are acceptable. See Instructions for Authors for complete listing. 䊐 Identify disk by providing: journal name (“JFAS”) corresponding author name/phone/fax, article title, and name of file on disk. ABSTRACT 䊐 Short summarization (1–3 paragraphs or less than 200 words) of purpose, methods, procedures, results, significance of findings. Do not include references or abbreviations. 䊐 Provide 3–5 key words or phrases (for indexing). TEXT 䊐 Introduction: Clear, concise background information and purpose of the study. 䊐 Materials, Methods, Apparatus and Procedures: Use sufficient detail to allow reproduction of results by peer readership. 䊐 Results: Present in logical sequence. 䊐 Discussion: Review primary objective, procedures, results, implications, limitations, and the relationship to other relevant studies. 䊐 Figures and Tables: Cite each figure and table in the text (in numerical order). 䊐 Clinical follow-up studies: Studies, particularly case reports, must present a minimum of 10 months. Greater than 1 year follow-up is preferred. 䊐 Number pages consecutively. REFERENCES 䊐 Verify all entries against original sources. 䊐 Number references consecutively in the order of their first use in the text (not alphabetically). 䊐 Refer to references in the text by number, and place reference numbers in line (not above the line) in the text within parentheses. 䊐 List references not identified in text as Additional References.

䊐 Abbreviate journal names according to standard format (Index Medicus). 䊐 Use the following standard citation format: Journals: 1. Mendicino RW, Orsini RC, Whitman SE, Cantanzariti AR. Fibular grove deepening for recurrent peroneal subluxation. J Foot Ankle Surg 40:252–263, 2001. Books: 2. Trevino SG. Disorders of the hallucal sesamoids. In Foot and Ankle Disorders, pp 379 –398, edited by MS Myerson, WB Saunders, Philadelphia, 2000. 䊐 List all authors (avoid “et al ”). FIGURES/ILLUSTRATIONS 䊐 Submit four sets of illustrations. 䊐 Do not clip, staple, tape, or mount prints. 䊐 Illustrations must be excellent quality, unmounted 5⬙ ⫻ 7⬙ or 8⬙ ⫻ 10⬙ glossy black and white prints. Black and white line drawings are acceptable only if done professionally. 䊐 Provide a back label for each figure, X-ray, and illustration, stating figure number (using Arabic, not Roman, numbers). Labeling should be done by typewriter. 䊐 Mark top of each figure (on back label). 䊐 Make sure that each figure is cited in the text in the same order in which figures are numbered. 䊐 Type (and clearly identify by number) legends for figures on a separate page, in paragraph form. 䊐 Verify that the figures and their legend numbers correspond. 䊐 Acknowledge original source(s) in legend, if figure is borrowed. 䊐 If borrowed, supply written permission to reproduce artwork from original copyright holder, including author and publisher. TABLES 䊐 Type each table double-spaced on a separated page and allow adequate margins. 䊐 Number tables consecutively, using Arabic (not Roman) numbers. 䊐 Make sure that each table is cited in the text in the same order in which table are numbered. 䊐 Place table number and title above a horizontal rule at top of table. 䊐 Place footnote (superscript “a,b,c,”) and explanations of abbreviations below a horizontal rule at bottom of table. 䊐 If borrowed, supply written permission to reproduce table from original copyright holder, including author and publisher. REPRINTS 䊐 Order on reprint order form; primary author will receive 5 gratis copies of issue in which article appears. 䊐 Please submit this form with four copies of your completed manuscript and four sets of original illustrations and one computer disk with all files to:
