ME1 Treatment-resistant depression W.A. Nolen. University Medical Center Groningen, Groningen, The Netherlands Background: Antidepressants are effective in only 50−60% of patients with major depressive disorder. For the further biological treatment of these patients various options are available. Major options are: (1) to continue with the same antidepressant for another 4−6 weeks, possibly at increased dose; (2) to switch to another antidepressant, from the same class or from another class; (3) to add another drug: e.g. another antidepressant, lithium or thyroid hormone; (4) to switch to a classical MAO-inhibitor; (5) to switch to electroconvulsive therapy (ECT), vagus nerve stimulation (VNS), rapid transcranial magnetic stimulation (r-TMS), or deep brain stimulation (DBS). Method: Several recent reviews and recent studies investigating the further treatment of patients who had not responded to a first trial with an antidepressant, are reviewed. Next, some cases are presented, with the intention that the participants discuss the further treatment options for these patients. Participants are also invited to present own cases.
