The book consists of two parts. The first part explains classic electrocardiography based on anatomical data, electrophysiological principles and heart cell metabolism. An attempt is made to explain each component of the different waves of the depolarization-repolarization cyles in normal and pathologic conditions. The authors condemn routine pattern-reading of ECGs without a deeper investigation or a logical approach. The second part covers more detailed concepts of heart cell metabolism and electronmicroscopy in various conditions, as well as discussing the importance of the polarizing treatment consisting of a diet low in sodium and high in potassium, as well as infusion of glucose, insulin and potassium. The work is the summary of the longstanding clinical and experimental experience of the Mexican National Cardiac Institute and contains several original ideas such as the polarizing effect, the septal barrier concept, the electrotone effect to explain pre-excitation, and classification and description of the bundle branch blocks. Three-quarters of the extensive bibliography list is to the work of the members of the Mexican Institute but no attempt is made to link each reference to the site in the book where it is discussed. The English translation of the original Spanish work contains 40 printing errors which are rather irritating. This is an unusual book on electrocardiography, quite different from the usual series of classic ECG books. Even though the way of thinking is very personal and sometimes philosophical, anyone who is involved with electrocardiography should at least be aware of the concepts put forward in this book.
