Measurements and Main Results. A multicenter study is comparing cryoablation under ultrasound guidance and rollerball electroablation in women with menorrhagia. All patients are premenopausal, menorrhagic with aPBAC diary score above 150, who failed medical therapy, have uterine volume less than 300 ml and uterine sound below 10 cm, and no abnormal endometrial pathology on ultrasound, endometrial biopsy, axld/or hysteroscopy. Stable intramural myomas did not exclude patients from participation. The first 20 patients who were treated with Her Option 4-minute first freeze and 6-minute second freeze were evaluated postoperatively by office hysteroscopy. They all had a grossly normal uterine cavity. In those who had monthly spotting or normal menstrual flow, no specific site of endometrium appeared to be more active than any other. Conclusion. The uterine cavity is maintained postoperatively in patients undergoing endometrial cryoablation with Her Option.

Posttrial data indicated that patients may undergo office endometrial ablation with Her Option with pretreatment with an NSAID and perioperative paracervical block anesthesia. All patients tolerated the procedure well. Conclusion. Uterine cryoblation may be performed successfully in an office setting with minimal anesthesia requirements in many women.

Objective. To assess the effect of Her Option cryoablation for AUB on quality of life. Measurements and Main Results. The median preoperative PBAC score for women treated with Her Option was 100 points higher than that for those having rollerball ablation, 450 versus 356.4. To the extent that preoperative bleeding may be associated with elevated postoperative bleeding, a bias against the Her Option group may be present. A PBAC score below 15 was achieved in 53% of Her Option-treated patients and 59% rollerball-treated women. In addition, 98% and 85%, respectively, would recommend the procedure to a friend. Finally, 76% and 66% of patients, respectively, experienced improved mood. Conclusion. Endometrial ablation by Her Option was somewhat more effective in improving quality of life than rollerball ablation.
