SIGNIFICANCE: This is the first study to provide anatomic information on the structural features of the pig cochlea using OCT. Although, studies have shown the utility of OCT imag-

P129 ing in rat cochlea, they posited that thick human bone would pose a technical challenge. The successful performance of intracochlear OCT imaging on pig cochlea, which more closely approximates human cochlear bone thickness, shows this technology can potentially guide microsurgical techniques on human cochleae in CI surgery.

Miniseminar: Balloon Sinuplasty: Who, When, Why, & Why Not Michael Friedman, MD (moderator); Howard L Levine, MD; William E Bolger, MD; James A Stankiewicz, MD Balloon sinuplasty is a controversial technique that has been incorporated into endoscopic sinus surgery. Clearly, long-term results are not available to assess what role this technique should have in the treatment of chronic rhinosinusitis. The goal of this miniseminar is to present the experience of four experts. They will define (1) patient selection criteria, (2) the results of

their two years of experience with the technique, (3) the arguments against the use of this technology and contraindications to its use, and (4) novel and innovative case presentations of its use. The panel will consist of members with varied opinions on its relative indications. After review of basic indications, the panel will review controversial case presentations, limited experience with the procedure under local anesthesia, revision cases with scarred frontal or sphenoid sinuses treated with balloon dilatation, and possible future innovation of the technique that may allow for in-office procedures. All complications encountered by the panelists will be reviewed and discussed.

decisions, present important considerations in the evaluation of contracts with third party payers, describe areas of importance when appealing denials, and discuss how to work with third party payers to resolve bundling issues. Each panelist will not only give a presentation related to his/her area of expertise but will also comment any of the topics presented so that varied opinions may be expressed. Questions and comments will be solicited from the audience at the end of the miniseminar.
