Nicolau syndrome is a rare vascular disease, occurring after injection of a drug, usually intramuscular. We report the case of a 32 year old woman with relapsing-remitting multiple sclerosis treated with intramuscular injections of glatiramer acetate who, after the last injection in abdomen, showed an ill-defined large plate in right hypochondriac region and flank of the mesogastrio, purplish color and infiltrated when touching (Fig. 1). Skin biopsy revealed epidermal changes and leukocytoclastic vasculitis with eosinophil isolated, indicative of toxicoderma plus lobular panniculitis with fat necrosis.
