NMR TECHNIQUES IN THE STUDY OF CARDIOVASCULAR STRUCTURE AND FUNCTION . By M . OSB.•",a:N and J. HASt:LGRO\·t:. Mount Kisco. NY: Futura Publishing Company. 198Il, 336 pp , $62 .00 This is a text for persons performing cardiac M RI, thinking about cardiac MRI or wanting an update on the state of cardiac MRI in 198Il. This text is a conscise, well-edited review of the ability and potential of cardiac MRI. All aspects of cardiac MRI are included from M RI basic physics to imal\inl\ and spectroscopy. The initial chapters discuss the basics of MRI physics and how the images are generated with cardiac gating. The Imaging section includes chapters on congenital abnormalities. myocardial ischemia and infarct imaging, fnnctional studies and the use of M RI contrast agents , The final section discusse s the early successes and problems with cardiac spectroscopy The spectroscopy section discusses the promising ahility of these techniques once they can be moved from the research area into the clinical arena. This book is ofinterest to anyone performing cardiac imaging. Radiologists, cardiologists, thoracic surgeons and physiologists may all be interested in reading this book . It surely belongs in the library of any department performing cardiac imaging.

SYNOPSIS OF CRITICAL CARE . 3rd ed . Edited by W,LLIAM J. S,BDALl>. Baltimore: Williams and Wilkins. 198Il. 334 pp. $29 .95 For the cynics who believe that good things cannot come in small packages, Dr. Sibhald has proven them wrung in the third edition of Synopsis oj Critica! Care . This multi-authored IKK.k covers critical care topics under six major headings: cardiovascular, pulmonary. renal, neurologic . trauma, and specific synd ro me s. The first chapter introduces critical care medicine . tracing its development on several continents, Quickly. our attention is directed to cardiac problems ran!\inl\ from chest pain to shock and CPR. The illustrations and diagrams clearly complement the text. The dtagnosis and approach to acute myocardial infarction is clear. and the subsequent chapter on cardiac arrhythmias is thorough . The chapter on shock (by Dr. Sihbald) is excellent and comprehensive . To discuss cardiopulmonary resuscitation in three pages of the next chapter is a most difficult assignment, but the author decided to

concentrate nil new developments-a wise decision . The discussion of respiratory failure is easy to follow and contains some very important information presented in tables and illustrations \of I\IKKI quality. The diagram and explanation on ventilation-perfusion mismatch is one of the clearest for a beginner that I have encountered. Mechanical support of respiration is easily understandable because of the excellent diagrams of circuits and apparatus. Algorithms in this and other chapters are very succinct and helpful. Radiographic reproductions are of high caliber and interpretable, Management of acute renal failure is clearly discussed. as are electrolyte disturbances. Even though the author recognizes that calcium is a major topic beyond the scope of the text. I would hope he reconsider its importance to be discussed with its other family members. phosphate and magnesium . The chapter. "Drug Dosage in Renal Failure," is elegant in its simplicity. I think that many who have tried to amid pharmacokinetics "ill read this chapter with little pain or difficulty. The neurologic sciences section contains a chapter on coma which is one of the more detailed chapters and may have been better served with a few more illustrations to clarify some of the neurologic lesions. There are nine clearly written and nicely referenced chapters devoted to each trauma system. The final section on specific syndromes brings together the multidisciplinary approach to critical care . Of note are excellent chapters on multi-organ failure and host defense mechanisms. The book is closed by an appendix which will serve as a handy reference for pertinent normal reference values. formulae and commonly used drugs, Synopsis of Critical Care holds no pretense in achieving what it set out to do ; present common ICU problems in a clear and concise manner to trainees and support sta ff in the critical care setting. I " '81Id encourage this IKK.k to students. junior residents. leu nurses. respiratory therapists . and other members of the ICU team.
